Description
Overview of the medicine
Sepra-S contains Esomeprazole, a proton pump inhibitor used to reduce stomach acid production. It treats conditions like GERD, ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
For GERD: 20 mg or 40 mg once daily for 4-8 weeks. For H. pylori eradication: 20 mg or 40 mg twice daily for 7-14 days.
Elderly
No dose adjustment generally required, but caution in patients with severe hepatic impairment.
Renal_impairment
No dose adjustment necessary for renal impairment.
How to Take
Take at least one hour before a meal. Swallow capsules/tablets whole; do not chew or crush. For patients with difficulty swallowing, capsules can be opened and the contents mixed with a small amount of water or soft food and consumed immediately.
Mechanism of Action
Esomeprazole selectively inhibits the H+/K+-ATPase enzyme system (the 'proton pump') in the gastric parietal cells, thereby blocking the final step of acid secretion.

Acid-suppressing effect begins within 1 hour.
Excretion
Mainly excreted in urine (80%) and feces (20%) as metabolites.

Side Effects
Contraindications
- Known hypersensitivity to Esomeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir.

Warfarin
Increased INR and prothrombin time; monitor patients closely.
Tacrolimus
Increased serum levels of tacrolimus; monitor concentrations.
Clopidogrel
Reduced antiplatelet effect of clopidogrel; generally avoid concomitant use.

Increased levels of methotrexate; consider temporary withdrawal of PPI.

Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. It is unknown if Esomeprazole is excreted in human milk; use with caution in nursing mothers.
