Description
Overview of the medicine
Seroxyn 50 mcg Inhalation Capsule contains a long-acting beta2-adrenergic agonist, used for the maintenance treatment of asthma and COPD. It helps to relax the muscles around the airways, making them wider and making it easier to breathe.
Uses & Indications
Dosage
Adults
1 inhalation (50 mcg) twice daily, approximately 12 hours apart. For exercise-induced bronchospasm, 1 inhalation (50 mcg) at least 30 minutes before exercise.
Elderly
No dosage adjustment is generally required for elderly patients.
Renal_impairment
No dosage adjustment is generally required for patients with renal impairment.
How to Take
For oral inhalation only. The capsule should be placed into a compatible dry powder inhaler device (e.g., Rotahaler) and the contents inhaled as directed. Do not swallow the capsule. Rinse mouth with water after use to minimize local side effects.
Mechanism of Action
Seroxyn acts locally in the lung as a bronchodilator by selectively stimulating beta2-adrenergic receptors on airway smooth muscle cells. This stimulation leads to an increase in intracellular cyclic AMP, causing smooth muscle relaxation and bronchodilation. It provides sustained bronchodilation for up to 12 hours.
Pharmacokinetics
Onset
Onset of action typically occurs within 10-20 minutes, with significant bronchodilation achieved within 30 minutes.
Excretion
Excreted primarily in feces (about 60%) and urine (about 25%) as metabolites within 72 hours.
Half life
The elimination half-life is approximately 5.5 hours after inhaled administration, but the clinical effect lasts longer.
Absorption
Minimal systemic absorption after inhalation, primarily acts locally in the lungs. Bioavailability is low due to extensive first-pass metabolism.
Metabolism
Extensively metabolized by hydroxylation, primarily by cytochrome P450 3A4 (CYP3A4) enzyme, to inactive metabolites.
Side Effects
Contraindications
- •Primary treatment of status asthmaticus or other acute episodes of asthma or COPD where intensive measures are required.
- •Hypersensitivity to the active substance or to any of the excipients (e.g., lactose).
Drug Interactions
Beta-blockers
May block the pulmonary effect of Seroxyn and produce severe bronchospasm in patients with asthma. Should generally be avoided.
MAOIs and TCAs
Monoamine Oxidase Inhibitors (MAOIs) and Tricyclic Antidepressants (TCAs) may potentiate the vascular effects of Seroxyn, leading to severe adverse cardiovascular effects. Use with extreme caution.
Other Sympathomimetics
Concomitant use with other sympathomimetic agents may potentiate adverse cardiovascular effects. Use with caution.
Strong CYP3A4 Inhibitors
Ketoconazole, ritonavir, atazanavir, clarithromycin, indinavir, itraconazole, nefazodone, nelfinavir, saquinavir, telithromycin may increase systemic exposure to Seroxyn, potentially leading to increased cardiovascular adverse effects (e.g., QTc prolongation).
Storage
Store in a dry place below 30°C (86°F). Protect from light and moisture. Keep out of reach of children. Do not refrigerate or freeze.
Overdose
Symptoms of overdose may include tachycardia, tremors, headache, palpitations, arrhythmias, hypertension or hypotension, and hypokalemia. Treatment is symptomatic and supportive. In severe cases, cardioselective beta-blockers may be considered with extreme caution.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. Caution advised during lactation as Seroxyn and its metabolites may be excreted in human milk, and the effects on the breastfed infant are unknown.
