Description
Overview of the medicine
Sertraline is a selective serotonin reuptake inhibitor (SSRI) antidepressant used to treat major depressive disorder, obsessive-compulsive disorder, panic disorder, post-traumatic stress disorder, premenstrual dysphoric disorder, and social anxiety disorder.
Uses & Indications
Dosage
Adults
Adults: For many indications, initial dose is 25 mg once daily, gradually increasing to a usual effective dose of 50 mg once daily. Maximum 200 mg/day.
Elderly
Elderly: Similar to adults, but dosage titration should be approached with caution, starting with a lower dose and slower escalation.
Renal_impairment
Renal impairment: No dosage adjustment is generally required for mild to moderate renal impairment. Use with caution in severe impairment.
How to Take
Administer orally once daily, in the morning or evening, with or without food.
Mechanism of Action
Sertraline selectively inhibits the reuptake of serotonin (5-HT) into presynaptic neurons in the central nervous system, thereby enhancing serotonergic neurotransmission.
Pharmacokinetics
Onset
Initial therapeutic effects may be seen within 1-2 weeks, but full antidepressant effects may take 4-6 weeks.
Excretion
Excreted in both urine (40-45%) and feces (40-45%) primarily as metabolites. Less than 0.2% is excreted unchanged in the urine.
Half life
Approximately 26 hours for sertraline and 62-104 hours for the active metabolite N-desmethylsertraline.
Absorption
Well absorbed orally; peak plasma concentrations achieved 4.5 to 8.4 hours after administration. Food slightly increases absorption.
Metabolism
Extensively metabolized in the liver via multiple cytochrome P450 isoforms (primarily CYP2C19, CYP2B6, CYP2D6, CYP3A4, CYP2C9) to inactive and active metabolites, mainly N-desmethylsertraline.
Side Effects
Contraindications
- Hypersensitivity to sertraline or any component of the formulation
- Concomitant use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of discontinuing an MAOI
- Concomitant use with pimozide
Drug Interactions
MAOIs
Concurrent use can lead to serious, sometimes fatal, serotonin syndrome.
Pimozide
Increased pimozide levels and cardiotoxicity.
Warfarin
Potentiation of anticoagulant effect, increased risk of bleeding.
NSAIDs/Aspirin
Increased risk of gastrointestinal bleeding.
Other serotonergic drugs (e.g., triptans, fentanyl, tramadol, St. John's wort)
Increased risk of serotonin syndrome.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include somnolence, nausea, vomiting, tachycardia, tremor, agitation, dizziness, and serotonin syndrome. Management is primarily supportive, including airway management, cardiovascular monitoring, and symptomatic treatment. Activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Small amounts excreted into breast milk; considered one of the preferred SSRIs for use during lactation due to low infant exposure, but monitor infant for adverse effects.
