Overview of the medicine
Sertlin 50 mg Tablet contains Sertraline, a selective serotonin reuptake inhibitor (SSRI) used to treat various mental health conditions including depression, obsessive-compulsive disorder, panic disorder, and social anxiety disorder. It works by increasing the levels of serotonin in the brain, which helps to improve mood and reduce anxiety.
Uses & Indications
Dosage
Adults
Initial dose is 50 mg once daily for MDD and OCD. For PD, PTSD, SAD, PMDD, initial dose is 25 mg once daily, increased to 50 mg after one week. Maximum dose for all indications is 200 mg once daily.
Elderly
Lower initial doses and slower titration may be considered for elderly patients due to potentially increased sensitivity.
Renal_impairment
No dosage adjustment is generally required for mild to moderate renal impairment. Caution is advised in severe renal impairment.
How to Take
Sertlin 50 mg Tablet should be taken orally once daily, either in the morning or evening, with or without food. It is recommended to take it at the same time each day.
Mechanism of Action
Sertraline selectively inhibits the reuptake of serotonin (5-HT) by the presynaptic neuron, thereby increasing the concentration of serotonin in the synaptic cleft and enhancing serotonergic neurotransmission. It has minimal effects on norepinephrine and dopamine reuptake.

Onset
Therapeutic effects typically begin within 2 to 4 weeks of treatment, though some symptom improvement may be seen earlier.
Excretion
Approximately 40-45% excreted in feces and 40-45% in urine, predominantly as metabolites. Less than 2% is excreted unchanged in urine.
Half life
Approximately 26 hours for sertraline and 62-104 hours for its active metabolite, N-desmethylsertraline.
Absorption
Well absorbed after oral administration, with peak plasma concentrations occurring 4.5 to 8.4 hours post-dose. Food slightly increases the absorption rate and AUC.
Metabolism
Extensively metabolized in the liver, primarily via CYP2D6, CYP2C19, CYP2B6, and CYP3A4, to its major inactive metabolite, N-desmethylsertraline.
Side Effects
Contraindications
- Concomitant use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of discontinuing an MAOI.
- Concomitant use with pimozide.
- Known hypersensitivity to sertraline or any component of the formulation.
Drug Interactions
MAOIs
Concurrent use may lead to serotonin syndrome, which can be life-threatening.
Pimozide
Increased pimozide levels and cardiotoxicity; contraindicated.
Warfarin
Increased prothrombin time/INR, potentially leading to increased bleeding risk.
CNS Depressants and Alcohol
May potentiate the effects of alcohol and other CNS depressants.

Pregnancy Category C. Sertraline should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in human breast milk in small amounts; caution should be exercised when administering to a nursing mother.
