Description
Overview of the medicine
Sevitan HTZ is a combination medication used to treat high blood pressure (hypertension). It contains Telmisartan, an angiotensin II receptor blocker (ARB), and Hydrochlorothiazide, a thiazide diuretic. This combination helps to effectively lower blood pressure by different mechanisms.
Uses & Indications
Dosage
Adults
Usually one tablet (Telmisartan 40 mg/Hydrochlorothiazide 12.5 mg) once daily. Dosage may be increased to 80 mg/12.5 mg or 80 mg/25 mg if blood pressure is not adequately controlled.
Elderly
No special dosage adjustment is generally required, but close monitoring is advised.
Renal_impairment
Not recommended for patients with severe renal impairment (creatinine clearance <30 mL/min). For moderate impairment, careful monitoring is needed.
How to Take
Take orally once daily, with or without food, at approximately the same time each day.
Mechanism of Action
Telmisartan selectively blocks the angiotensin II type 1 (AT1) receptors, preventing angiotensin II from binding and causing vasoconstriction and aldosterone secretion. This leads to vasodilation and reduced sodium and water retention. Hydrochlorothiazide acts on the renal distal convoluted tubule to inhibit sodium and chloride reabsorption, thereby increasing the excretion of sodium, chloride, and water, which reduces plasma volume and blood pressure.
Pharmacokinetics
Onset
Antihypertensive effect of Telmisartan: noticeable within 3 hours, maximum effect within 4-8 weeks. Hydrochlorothiazide: Onset of diuresis within 2 hours.
Excretion
Telmisartan: Excreted almost exclusively in feces via biliary excretion. Hydrochlorothiazide: Primarily eliminated unchanged by renal excretion.
Half life
Telmisartan: ~24 hours. Hydrochlorothiazide: 5.6 to 14.8 hours.
Absorption
Telmisartan: Rapidly absorbed, absolute bioavailability ~42% (80mg dose). Hydrochlorothiazide: Rapidly absorbed, bioavailability ~65-75%.
Metabolism
Telmisartan: Metabolized by conjugation to form an acylglucuronide, which is inactive. Hydrochlorothiazide: Not metabolized.
Side Effects
Contraindications
- •Hypersensitivity to Telmisartan, Hydrochlorothiazide, or any sulfonamide-derived drugs.
- •Pregnancy (especially second and third trimesters).
- •Severe hepatic impairment or biliary obstructive disorders.
- •Anuria or severe renal impairment (creatinine clearance <30 mL/min).
- •Refractory hypokalemia, hyponatremia, hypercalcemia, and symptomatic hyperuricemia.
Drug Interactions
Digoxin
Hydrochlorothiazide-induced hypokalemia or hypomagnesemia may predispose to digitalis toxicity.
Lithium
Increased serum lithium concentrations and toxicity. Monitor lithium levels.
Corticosteroids, ACTH
May intensify electrolyte depletion, particularly hypokalemia.
Other antihypertensives
Additive hypotensive effect.
NSAIDs (Non-Steroidal Anti-inflammatory Drugs)
May reduce the diuretic and antihypertensive effects of hydrochlorothiazide and telmisartan. May increase the risk of renal function deterioration.
Potassium-sparing diuretics, potassium supplements, or potassium-containing salt substitutes
May lead to increased serum potassium levels, especially in patients with renal impairment.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include hypotension, dizziness, tachycardia, bradycardia (less common with telmisartan), and electrolyte imbalances. Management involves symptomatic and supportive treatment, including monitoring vital signs, electrolyte levels, and renal function. Intravenous fluids may be administered for hypotension.
Pregnancy & Lactation
Sevitan HTZ is contraindicated during the second and third trimesters of pregnancy due to the risk of fetal injury and death. It is not recommended during the first trimester. Due to the potential for adverse effects on the nursing infant, it is generally not recommended during lactation.
