Description
Overview of the medicine
Sevoflurane is a halogenated ether volatile liquid used as an inhalation general anesthetic for induction and maintenance of anesthesia in adults and pediatric patients.
Uses & Indications
Dosage
Adults
Induction: 0.5-5% (up to 8% for rapid induction). Maintenance: 0.5-3% with N2O/O2. Individualized dosage by trained anesthetist.
Elderly
Lower concentrations typically required due to reduced MAC with age.
Renal_impairment
Use with caution in patients with renal impairment due to potential fluoride ion production; monitor renal function.
How to Take
Administered by inhalation via a calibrated vaporizer, typically with oxygen and/or nitrous oxide.
Mechanism of Action
Modulates neuronal excitability, primarily by enhancing GABA-A receptor activity, inhibiting NMDA receptors, and affecting potassium channels, leading to central nervous system depression.
Pharmacokinetics
Onset
Rapid (1-2 minutes for induction).
Excretion
Primarily excreted unchanged via the lungs; metabolites excreted renally.
Half life
Short elimination half-life, rapid emergence.
Absorption
Rapid uptake from lungs.
Metabolism
Minimal metabolism (about 5%) in the liver via CYP2E1, producing inorganic fluoride and hexafluoroisopropanol.
Side Effects
Contraindications
- Known hypersensitivity to sevoflurane or other halogenated agents
- Known or suspected malignant hyperthermia susceptibility
- History of unexplained moderate to severe hepatic dysfunction with previous halogenated anesthetics
- Patients with intracranial hypertension
Drug Interactions
Opioids
May reduce sevoflurane requirements and increase respiratory depression.
Beta-blockers
May increase risk of bradycardia and hypotension.
Nitrous Oxide
May decrease MAC (Minimum Alveolar Concentration) and enhance anesthetic effect.
Non-depolarizing muscle relaxants
Potentiates their effect, requiring lower doses.
Storage
Store at room temperature (15-30°C or 59-86°F). Protect from light. Keep container tightly closed when not in use. Do not freeze.
Overdose
Overdose leads to excessive depth of anesthesia, severe hypotension, and respiratory depression. Management involves reducing sevoflurane concentration, increasing inspired oxygen, and providing supportive cardiovascular and respiratory care.
Pregnancy & Lactation
Pregnancy Category B. Use during pregnancy only if clearly needed. Excreted in breast milk, use with caution. Consult a healthcare professional.
