Description
Overview of the medicine
Sigtil 50 mg Tablet contains Sitagliptin, an oral antidiabetic medicine used to lower blood sugar levels in adults with type 2 diabetes mellitus.
Uses & Indications
Dosage
Adults
Usually 100 mg once daily. For moderate renal impairment (CrCl ≥30 to <50 mL/min), 50 mg once daily is recommended. For severe renal impairment (CrCl <30 mL/min) or ESRD requiring dialysis, 25 mg once daily is recommended.
Elderly
No dose adjustment generally required based on age alone; however, renal function should be assessed.
Renal_impairment
Moderate renal impairment (CrCl ≥30 to <50 mL/min): 50 mg once daily. Severe renal impairment (CrCl <30 mL/min) or ESRD requiring dialysis: 25 mg once daily.
How to Take
Administer orally once daily, with or without food.
Mechanism of Action
Sitagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor that enhances the activity of incretin hormones (GLP-1 and GIP), leading to increased insulin synthesis and release from pancreatic beta cells and decreased glucagon secretion from alpha cells, thereby lowering blood glucose.
Pharmacokinetics
Onset
Peak plasma concentrations reached in 1-4 hours.
Excretion
Primarily renal (79% unchanged), with some fecal excretion.
Half life
Approximately 12.4 hours.
Absorption
Rapidly absorbed, bioavailability approximately 87%. Peak plasma concentrations reached in 1-4 hours.
Metabolism
Minimally metabolized, primarily by CYP3A4 and to a lesser extent by CYP2C8.
Side Effects
Contraindications
- Hypersensitivity to sitagliptin or any component of the formulation.
- Patients with a history of serious hypersensitivity reaction to sitagliptin (e.g., anaphylaxis, angioedema).
Drug Interactions
Digoxin
Monitor digoxin levels if coadministered, as sitagliptin may slightly increase digoxin plasma concentrations.
Cyclosporine
Minimal interaction, no significant dose adjustment needed.
Storage
Store below 30°C in a dry place, away from light and moisture.
Overdose
In case of overdose, symptomatic and supportive treatment is recommended. Sitagliptin is modestly dialyzable.
Pregnancy & Lactation
Use during pregnancy only if clearly needed and potential benefits outweigh risks. Caution is advised during lactation, as sitagliptin is excreted in animal milk.

Lab Monitoring
- HbA1c levels (regularly to assess glycemic control).
- Renal function (e.g., CrCl) prior to initiation and periodically thereafter.
Doctor Notes
- Consider dose adjustment for renal impairment based on CrCl.
- Counsel patients on signs of pancreatitis (e.g., persistent severe abdominal pain radiating to the back) and hypersensitivity reactions (e.g., rash, angioedema).
- Advise patients about the importance of diet and exercise in diabetes management.
Patient Guidelines
- Take as directed by your doctor, usually once daily.
- Do not stop taking without consulting your doctor.
- Maintain diet and exercise regimen recommended by your doctor.
- Report any unusual side effects, especially severe joint pain or signs of pancreatitis (e.g., persistent severe abdominal pain).

Driving Precautions
Generally no effect on ability to drive or operate machinery. However, if symptoms of hypoglycemia (e.g., dizziness) occur, caution is advised, especially if used with insulin or sulfonylureas.
Lifestyle Advice
