Description
Overview of the medicine
Silclog (Cilostazol) is a medication used to reduce the symptoms of intermittent claudication, which is leg pain caused by too little blood flow during exercise. It works by improving blood flow to the legs.
Uses & Indications
Dosage
Adults
The recommended dosage is 100 mg twice daily, taken at least 30 minutes before or 2 hours after breakfast and dinner. A 50 mg twice daily dose may be considered in patients receiving concomitant inhibitors of CYP3A4 or CYP2C19.
Elderly
No specific dosage adjustment is required based on age, but caution is advised due to potential for increased sensitivity.
Renal_impairment
Use with caution in severe renal impairment (creatinine clearance ≤25 mL/min); dosage reduction may be necessary.
How to Take
Take orally with water, at least 30 minutes before or 2 hours after breakfast and dinner. Do not crush or chew the tablet.
Mechanism of Action
Cilostazol is a phosphodiesterase-3 (PDE3) inhibitor that suppresses platelet aggregation and causes vasodilation. It increases intracellular cyclic AMP (cAMP) in platelets and vascular smooth muscle, leading to inhibition of platelet aggregation and vasodilation.
Pharmacokinetics
Onset
Clinical effects may take 2-4 weeks to be evident, maximal effects in 12 weeks.
Excretion
Excreted primarily in urine (74%) and feces (20%) as metabolites.
Half life
Approximately 11-13 hours (for cilostazol and its active metabolites).
Absorption
Rapidly absorbed after oral administration. High-fat meals can increase absorption.
Metabolism
Extensively metabolized in the liver, primarily by CYP3A4 and CYP2C19, to two active metabolites.
Side Effects
Contraindications
- Congestive heart failure of any severity (Black Box Warning).
- Known hypersensitivity to cilostazol or any component of the formulation.
- Hemostatic disorders or active pathological bleeding (e.g., active peptic ulceration, intracranial hemorrhage).
- Patients with severe kidney or liver impairment.

Increased risk of bleeding.
Antiplatelet agents (e.g., aspirin, clopidogrel)
Increased risk of bleeding.
