Description
Overview of the medicine
Silofast 4 mg Capsule contains Silodosin, an alpha-1 adrenoceptor antagonist used to improve urination in men with benign prostatic hyperplasia (BPH). It works by relaxing muscles in the prostate and bladder neck, making it easier to urinate.
Uses & Indications
Dosage
Adults
The recommended dose is 8 mg orally once daily with a meal. Some patients may be initiated on 4 mg once daily with a meal based on clinical judgment.
Elderly
No specific dose adjustment is required for elderly patients (≥65 years) with normal renal function; however, careful monitoring is advised.
Renal_impairment
For moderate renal impairment (CrCl 30-50 mL/min), the recommended dose is 4 mg orally once daily with a meal. Silodosin is contraindicated in patients with severe renal impairment (CrCl <30 mL/min).
How to Take
Take orally once daily with a meal, preferably at the same time each day. Swallow the capsule whole with water; do not crush, chew, or open it.
Mechanism of Action
Silodosin selectively blocks alpha-1A adrenoceptors located in the prostate, bladder neck, and prostatic urethra. This blockade causes relaxation of the smooth muscles in these tissues, thereby reducing the dynamic component of bladder outflow obstruction associated with BPH and improving urine flow.
Pharmacokinetics
Onset
Symptom improvement may begin within 1 week; full effects are typically seen within 2-4 weeks.
Excretion
Approximately 33.5% of the dose is excreted in urine and 54.9% in feces over 7 days.
Half life
The terminal elimination half-life is approximately 13.3 hours for silodosin and 24 hours for its active metabolite (KMD-3213G).
Absorption
Rapidly absorbed. Peak plasma concentrations reached in 2.5-4 hours. Oral bioavailability is approximately 32%. Absorption is slightly increased when taken with food.
Metabolism
Extensively metabolized, primarily in the liver, through glucuronidation (UGT1A2), alcohol dehydrogenase, and aldehyde dehydrogenase pathways. The major circulating metabolite is KMD-3213G (glucuronide conjugate of silodosin).
Side Effects
Contraindications
- •Hypersensitivity to silodosin or any component of the formulation
- •Severe renal impairment (CrCl <30 mL/min)
- •Severe hepatic impairment
- •Concomitant use with strong CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin, ritonavir)
- •Concomitant use with other alpha-adrenergic blocking agents due to additive hypotensive effects.
Drug Interactions
Antihypertensives
May increase the risk of orthostatic hypotension due to additive hypotensive effects. Adjust dosage of antihypertensives if necessary.
Other Alpha-blockers
Concurrent use is not recommended due to the potential for additive hypotensive effects.
PDE5 Inhibitors (e.g., sildenafil, tadalafil)
Increased risk of symptomatic orthostatic hypotension. Use with caution.
Moderate CYP3A4 Inhibitors (e.g., diltiazem, erythromycin)
Use with caution; consider a lower starting dose of silodosin.
Strong CYP3A4 Inhibitors (e.g., ketoconazole, clarithromycin, ritonavir)
Significantly increases silodosin exposure; contraindicated.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Overdose of Silodosin can lead to severe hypotension and compensatory tachycardia. Management should be supportive, including maintaining cardiovascular stability with intravenous fluids and vasopressors if necessary. Due to extensive protein binding, dialysis is unlikely to be beneficial in removing silodosin.
Pregnancy & Lactation
Silodosin is not indicated for use in women. Its safety in pregnancy and lactation has not been established as it is only for male use in benign prostatic hyperplasia.
