Uses & Indications
Dosage

Not typically used for adults in this strength; refer to higher strength formulations.
Elderly
Not typically used for elderly in this strength; refer to higher strength formulations.
Renal_impairment

How to Take
Administer orally. Shake the bottle well before use. Use a measuring spoon or oral syringe to ensure accurate dosing. Can be taken with or without food.
Mechanism of Action
Paracetamol's exact mechanism is not fully understood, but it is believed to act primarily by inhibiting prostaglandin synthesis in the central nervous system, leading to its analgesic and antipyretic effects. It has weak anti-inflammatory effects peripherally.
Pharmacokinetics
Onset
30-60 minutes for analgesic/antipyretic effect.
Excretion
Mainly excreted in the urine as glucuronide and sulfate conjugates. Less than 5% is excreted unchanged.
Half life
1-4 hours (typically 2-3 hours) in adults; slightly longer in neonates, shorter in children.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract, with peak plasma concentrations occurring within 10-60 minutes after oral administration.
Metabolism
Primarily metabolized in the liver via glucuronidation and sulfation. A small amount is metabolized by cytochrome P450 to a highly reactive intermediate (NAPQI), which is normally detoxified by glutathione.
Side Effects
Contraindications
- •Known hypersensitivity to Paracetamol or any other component of the formulation.
- •Severe hepatic impairment or active liver disease.
Drug Interactions
Alcohol
Increased risk of hepatotoxicity with chronic excessive alcohol intake.
Warfarin
May enhance anticoagulant effect, increasing risk of bleeding with prolonged high-dose use.
Cholestyramine
Reduces paracetamol absorption if taken within one hour.
Metoclopramide, Domperidone
Increases paracetamol absorption rate.
Storage
Store below 30°C in a dry place, away from light. Do not freeze.
Overdose
Overdose of Paracetamol can cause severe, potentially fatal liver damage. Symptoms may include nausea, vomiting, abdominal pain, and sweating, which may appear 24-48 hours after ingestion. Seek immediate medical attention. Treatment includes gastric lavage, activated charcoal, and N-acetylcysteine (NAC) administration.
Pregnancy & Lactation
Paracetamol is generally considered safe during pregnancy and lactation when used at recommended doses for the shortest possible duration. Consult a doctor before use.
