Overview of the medicine
Silpol 500 mg tablet contains Ciprofloxacin, a broad-spectrum fluoroquinolone antibiotic used to treat various bacterial infections including urinary tract, respiratory, skin, bone, joint, and gastrointestinal infections. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
Standard dose is 250-750 mg twice daily, depending on the type and severity of infection. For most infections, 500 mg twice daily for 7-14 days.
Elderly
No specific dose adjustment is needed based solely on age, but renal function should be assessed. Dosage should be based on infection type and severity.
Renal_impairment
For creatinine clearance 30-60 mL/min, administer 250-500 mg every 12 hours. For creatinine clearance <30 mL/min, administer 250-500 mg every 18-24 hours.
How to Take
Silpol 500 mg tablets should be swallowed whole with a glass of water, with or without food. Do not chew or crush the tablet. It is best to take it at approximately the same times each day. Do not take with dairy products or calcium-fortified juices.
Mechanism of Action
Ciprofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, enzymes essential for bacterial DNA replication, transcription, repair, and recombination. This leads to the inhibition of bacterial growth and ultimately cell death.

Onset
Within 30-60 minutes (oral)
Excretion
Primarily excreted unchanged in the urine (about 50-70% of an oral dose) and, to a lesser extent, via feces.
Half life
Approximately 3-5 hours in adults with normal renal function.
Absorption
Rapidly and well absorbed from the gastrointestinal tract, with bioavailability of approximately 70-80%. Peak plasma concentrations are achieved 1-2 hours after oral administration.
Metabolism
Approximately 15-20% of the dose is metabolized in the liver to less active metabolites.
Side Effects
Contraindications
- •Hypersensitivity to ciprofloxacin or other fluoroquinolones.
- •Concomitant administration with tizanidine.
- •History of tendinitis or tendon rupture associated with fluoroquinolone use.
Drug Interactions
Warfarin
May enhance the anticoagulant effect of warfarin, increasing bleeding risk. Monitor INR.
Tizanidine
Increased risk of hypotension and sedation. Concomitant use is contraindicated.
Theophylline
Increased plasma levels of theophylline, leading to toxicity. Monitor theophylline levels.
Corticosteroids
Increased risk of tendinitis and tendon rupture.
Antacids containing aluminum or magnesium, sucralfate, iron or zinc supplements
Reduce absorption of ciprofloxacin. Take ciprofloxacin at least 2 hours before or 6 hours after these agents.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of an overdose, symptomatic and supportive treatment should be initiated. Gastric lavage may be considered. Maintain adequate hydration. Ciprofloxacin is poorly removed from the body by hemodialysis or peritoneal dialysis.
Pregnancy & Lactation
Pregnancy Category C. Ciprofloxacin should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in human milk; therefore, a decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
