Description
Overview of the medicine
Silver Sulfadiazine is a topical antimicrobial agent used to prevent and treat wound infections, particularly in second and third-degree burns.
Uses & Indications
Dosage
Adults
Apply a thin layer (approx. 1-2 mm thick) to the affected area once or twice daily, covering the entire wound surface.
Elderly
No specific dose adjustment required, but monitor for systemic absorption.
Renal_impairment
Use with caution in severe renal impairment, monitor for systemic toxicity if absorbed.
How to Take
Cleanse the wound thoroughly. Apply the cream to the affected areas using a sterile gloved hand or applicator. Reapply if removed by patient activity.
Mechanism of Action
It acts on the bacterial cell wall and membrane, leading to damage and cell death. The sulfadiazine component also interferes with bacterial folic acid synthesis.
Pharmacokinetics
Onset
Immediate local antibacterial action.
Excretion
Systemically absorbed sulfadiazine and its metabolites are primarily excreted in urine.
Half life
Not well defined for topical use; systemically absorbed sulfadiazine has a half-life of 10-12 hours.
Absorption
Minimal systemic absorption through intact skin. Absorption may increase in large burn areas.
Metabolism
Systemically absorbed sulfadiazine is metabolized in the liver.
Side Effects
Contraindications
- Hypersensitivity to silver sulfadiazine or sulfonamides.
- Premature infants or neonates (<2 months of age) due to risk of kernicterus.
- Pregnancy at term, or in women who are breast-feeding.
Drug Interactions
Cimetidine
Increased risk of leukopenia.
Enzymatic debriding agents
May be inactivated by silver. Should not be used concomitantly.
Storage
Store below 30°C. Protect from light and moisture. Do not freeze.
Overdose
Systemic overdose is unlikely with topical application. If large areas are treated for prolonged periods, systemic absorption may lead to sulfonamide toxicity (e.g., renal impairment, blood dyscrasias). Treatment is supportive, including discontinuing the drug and managing symptoms.
Pregnancy & Lactation
Avoid during late pregnancy and in breast-feeding mothers due to potential risk of kernicterus in infants. Consult a doctor.
