Description
Overview of the medicine
SMC ORSaline is an oral rehydration salt formulation used to treat and prevent dehydration caused by diarrhea or excessive fluid loss. It replenishes essential electrolytes and fluids lost from the body.
Uses & Indications
Dosage
Adults
For mild to moderate dehydration, 1 sachet dissolved in 500 ml (half liter) of water, consumed as needed, typically 1 to 3 liters over 24 hours. For severe dehydration, medical supervision required.
Elderly
Same as adult dosage, monitor closely for fluid overload if cardiac or renal issues present.
Renal_impairment
Use with caution and monitor electrolyte levels, especially potassium. Consultation with a physician is recommended.
How to Take
Dissolve the entire contents of one sachet in 500 ml (half liter) of clean drinking water. Stir until completely dissolved. Do not add extra sugar. Do not mix with milk, soup, or fruit juice. Administer orally.
Mechanism of Action
ORSaline works by utilizing the co-transport of sodium and glucose in the small intestine, which facilitates the absorption of water and electrolytes, thereby rehydrating the body.
Pharmacokinetics
Onset
Rapid (within minutes to hours depending on hydration status).
Excretion
Excess electrolytes and water are excreted primarily via kidneys.
Half life
Not applicable as it's a salt solution, not a drug with a distinct half-life.
Absorption
Components (glucose, sodium, potassium, chloride, citrate) are readily absorbed from the gastrointestinal tract.
Metabolism
Not metabolized in the conventional sense; components are absorbed and utilized or excreted.
Side Effects
Contraindications
- Intestinal obstruction
- Paralytic ileus
- Severe vomiting making oral intake impossible
- Glucose malabsorption
- Kidney failure (anuria or severe oliguria)
Drug Interactions
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Overdose is unlikely with proper preparation and administration. Excessive intake, especially in patients with impaired renal function, could lead to hypernatremia or hyperkalemia. Management includes discontinuing ORS and correcting electrolyte imbalances.
Pregnancy & Lactation
Safe to use during pregnancy and lactation. Dehydration is more dangerous than ORS. Consult a doctor for severe cases.

WHO Essential Medicine
YesClinical Trials
Extensive clinical trials since the 1970s have proven the efficacy of ORS, leading to its global adoption. Specific trials for SMC ORSaline usually involve bioequivalence to WHO standards.
Lab Monitoring
- In severe dehydration or compromised renal function, monitor serum electrolytes (sodium, potassium) and renal function tests (creatinine, BUN).
- Blood glucose levels (especially in infants or diabetics).
Doctor Notes
- Emphasize proper preparation and administration to patients/caregivers.
- Monitor for signs of severe dehydration, in which case IV fluids may be necessary.
- Advise continued feeding, especially breastfeeding, during diarrheal illness.
Patient Guidelines
- Always use clean, boiled, and cooled water to prepare ORS solution.
- Prepare fresh solution daily and discard any unused portion after 24 hours.
- Do not store solution in a refrigerator for more than 24 hours.
- Continue breastfeeding for infants and young children.
- Seek medical attention if diarrhea worsens, blood appears in stool, or signs of severe dehydration persist.
Missed Dose Advice
ORS is taken as needed to replace fluid loss. There isn't a 'missed dose' in the traditional sense. Continue taking as soon as fluid loss occurs.
Driving Precautions
Does not impair ability to drive or operate machinery.
Lifestyle Advice
- Maintain good hygiene, including hand washing, to prevent further spread of infection.
- Consume safe, clean food and water.
- Rest as much as possible.
