Description
Overview of the medicine
Solbion 100 mg injection is a vitamin B1 (Thiamine Hydrochloride) supplement used to treat and prevent thiamine deficiency. Thiamine is crucial for carbohydrate metabolism, nerve function, and cardiovascular health.
Uses & Indications
Dosage
Adults
For severe deficiency, 100 mg IM daily for several days, then switch to oral supplementation. For Wernicke-Korsakoff syndrome, 100-200 mg IM/IV three times daily until symptoms improve.
Elderly
No specific dosage adjustment for elderly patients, but consider age-related physiological changes and potential renal impairment.
Renal_impairment
No specific dosage adjustment is generally required, but close monitoring is advised in severe renal impairment.
How to Take
Administer by deep intramuscular (IM) injection. Intravenous (IV) administration should be done slowly and cautiously due to the risk of hypersensitivity reactions.
Mechanism of Action
Thiamine is converted in the body to thiamine pyrophosphate (TPP), an active coenzyme. TPP is essential for various metabolic processes, including carbohydrate metabolism (e.g., pyruvate dehydrogenase and alpha-ketoglutarate dehydrogenase complexes) and the pentose phosphate pathway. It plays a vital role in energy production and nerve impulse transmission.
Pharmacokinetics
Onset
Within minutes to hours for symptomatic relief depending on the severity of deficiency and route of administration (faster with IV).
Excretion
Excreted primarily via the kidneys as metabolites or unchanged thiamine, especially after high doses.
Half life
Biphasic elimination with an initial half-life of 0.35 hours and a terminal half-life of 9-18 days.
Absorption
Rapidly absorbed after intramuscular (IM) injection. Plasma protein binding is minimal. Transport across the intestinal wall is dose-dependent.
Metabolism
Primarily metabolized in the liver to various inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to thiamine or any components of the injection
Drug Interactions
Fluorouracil
May inhibit the phosphorylation of thiamine to its active form, potentially exacerbating thiamine deficiency.
IV solutions containing sulfites
Sulfites can degrade thiamine, reducing its effectiveness. Avoid co-administration or monitor carefully.
Storage
Store below 25-30°C. Protect from light and moisture. Do not freeze.
Overdose
Thiamine has a wide margin of safety, and overdose is rare with typical therapeutic use. Extremely high doses may cause symptoms like nausea, warmth sensation, weakness, and sweating. Management is supportive and symptomatic.
Pregnancy & Lactation
Thiamine is generally considered safe during pregnancy and lactation when administered in recommended doses. Consult a doctor before use.
