Sonexim 200 mg Capsule is a proton pump inhibitor (PPI) containing Esomeprazole, used to reduce stomach acid production. It treats conditions such as gastroesophageal reflux disease (GERD), peptic ulcers, and Zollinger-Ellison syndrome by significantly decreasing the amount of acid produced in the stomach.

Dosage
Adults
The usual dosage for Esomeprazole is 20 mg or 40 mg once daily. A 200 mg capsule is highly atypical and should only be used under strict medical supervision for very specific, severe hypersecretory conditions like refractory Zollinger-Ellison syndrome, with individual dose titration. Elderly
No general dosage adjustment is necessary for elderly patients, but caution is advised, especially with a high strength like 200 mg. Consult a physician for specific guidance.

How to Take

Mechanism of Action
Esomeprazole, an S-isomer of omeprazole, specifically inhibits the H+/K+-ATPase (proton pump) in the gastric parietal cells. By binding irreversibly to the proton pump, it blocks the final step of acid secretion, leading to a profound and long-lasting reduction in gastric acid production.

Absorption
Rapidly absorbed after oral administration. Bioavailability is about 64% after a single dose and increases to 89% after repeated once-daily dosing. Peak plasma concentrations are reached within 1-2 hours.
Metabolism
Primarily metabolized in the liver by the cytochrome P450 enzyme system, mainly CYP2C19 (responsible for the majority of metabolism) and CYP3A4. Extensive first-pass metabolism occurs.
Side Effects
Contraindications
- •Hypersensitivity to esomeprazole, substituted benzimidazoles (e.g., omeprazole, lansoprazole, pantoprazole), or any component of the formulation.
- •Concomitant use with nelfinavir due to significant reduction in nelfinavir plasma levels.
Drug Interactions
Digoxin
Increased absorption of digoxin due to altered gastric pH; monitor digoxin levels and adjust dosage as needed.
Warfarin
Increased INR and prothrombin time; monitor closely. Dosage adjustment of warfarin may be necessary.
Clopidogrel
Esomeprazole may reduce the antiplatelet effect of clopidogrel by inhibiting CYP2C19. Concomitant use should be avoided.
Methotrexate
May increase serum levels of methotrexate; consider temporary PPI withdrawal or close monitoring if high-dose methotrexate is administered.
Atazanavir, Nelfinavir
Reduced absorption of antiretrovirals due to increased gastric pH; concomitant use is generally contraindicated or requires careful monitoring and alternative therapies.

Overdose
Symptoms of Esomeprazole overdose are generally mild and may include gastrointestinal disturbances (e.g., diarrhea, abdominal pain), fatigue, headache, and confusion. Treatment is symptomatic and supportive. There is no specific antidote for esomeprazole overdose. Hemodialysis is not effective.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed and the potential benefits outweigh the potential risks to the fetus. Esomeprazole is excreted in human breast milk; caution is advised when administering to a nursing mother.
