Description
Overview of the medicine
Flucloxacillin is a narrow-spectrum beta-lactam antibiotic of the penicillin class. It is used to treat bacterial infections caused by susceptible Gram-positive bacteria.
Uses & Indications
Dosage
Adults
250-500 mg every 6 hours, usually for 5-7 days or longer depending on infection severity.
Elderly
Same as adults, but dose adjustment may be required in severe renal impairment.
Renal_impairment
Dose reduction needed for severe renal impairment (e.g., CrCl < 10 mL/min).
How to Take
Take orally, preferably on an empty stomach (at least 30 minutes before food or 2 hours after food) to ensure optimal absorption.
Mechanism of Action
Inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to cell lysis and death.

Onset
Rapid (within 30-60 minutes).
Excretion
Primarily excreted unchanged in urine by renal tubular secretion and glomerular filtration.
Half life
Approximately 0.5-1 hour.
Absorption
Well absorbed orally, especially on an empty stomach. Peak plasma levels usually occur 1-2 hours after oral administration.
Metabolism
Partially metabolized in the liver to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to penicillin antibiotics or any component of the formulation.
- History of flucloxacillin-associated jaundice/hepatic dysfunction.
Drug Interactions
Warfarin
May potentiate anticoagulant effect, requiring closer INR monitoring.
Probenecid
Increases and prolongs flucloxacillin blood levels.
Methotrexate
Increased methotrexate toxicity due to reduced renal excretion.
Oral Contraceptives
May reduce efficacy of oral contraceptives (advise additional contraception).
Storage

Overdose
Symptomatic and supportive treatment. No specific antidote. Hemodialysis is not effective.
