Overview of the medicine
Staxiclav 250 mg Tablet is a combination antibiotic containing Amoxicillin and Clavulanic Acid. It is used to treat a wide range of bacterial infections by killing bacteria and preventing them from becoming resistant to the antibiotic.
Uses & Indications
Dosage
Adults
One Staxiclav 250 mg/125 mg tablet three times daily (every 8 hours) for 7-10 days, or as directed by physician. Dosage may vary based on infection severity and site.
Elderly
Same as adult dose, but monitor renal function and adjust if necessary.
Renal_impairment
Dose adjustment required. For CrCl 10-30 mL/min, 250 mg/125 mg twice daily. For CrCl <10 mL/min, 250 mg/125 mg once daily. Hemodialysis patients: 250 mg/125 mg once daily, plus an additional dose during and at the end of dialysis.
How to Take
Take orally at the start of a meal to reduce potential gastrointestinal intolerance and optimize absorption of clavulanic acid. Swallow whole with water; do not chew or crush.
Mechanism of Action
Amoxicillin, a beta-lactam antibiotic, inhibits bacterial cell wall synthesis. Clavulanic acid is a beta-lactamase inhibitor that protects amoxicillin from degradation by beta-lactamase enzymes produced by resistant bacteria, thereby extending amoxicillin's antibacterial spectrum.

Excretion
Primarily excreted unchanged in the urine (amoxicillin) and as metabolites (clavulanic acid).
Half life
Amoxicillin: approximately 1-1.5 hours; Clavulanic Acid: approximately 1 hour.
Absorption
Both amoxicillin and clavulanic acid are well absorbed from the gastrointestinal tract after oral administration. Absorption of clavulanic acid is enhanced when taken with food.
Metabolism
Amoxicillin is minimally metabolized; clavulanic acid is extensively metabolized to inactive metabolites.
Side Effects
Contraindications
- History of severe hypersensitivity reaction (e.g., anaphylaxis, Stevens-Johnson syndrome) to any penicillin or other beta-lactam antibiotics.
- History of amoxicillin/clavulanate-associated jaundice or hepatic dysfunction.
Drug Interactions
Probenecid
Increases and prolongs amoxicillin blood levels; not recommended for concomitant use.
Allopurinol
Increased incidence of skin rash when co-administered with amoxicillin.
Methotrexate
Reduced renal tubular secretion of methotrexate, increasing its plasma concentration and potential toxicity.
Oral contraceptives
May reduce the efficacy of oral contraceptives; advise using an additional non-hormonal birth control method.
Oral anticoagulants (e.g., Warfarin)
May prolong prothrombin time (INR). Increased risk of bleeding.

Symptoms of overdose include gastrointestinal symptoms (e.g., abdominal pain, vomiting, diarrhea) and disturbances of fluid and electrolyte balance. Renal impairment may lead to convulsions. Treatment is symptomatic and supportive. Amoxicillin can be removed from circulation by hemodialysis.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but adequate and well-controlled studies in pregnant women are lacking. Use during pregnancy only if clearly needed. Both amoxicillin and clavulanic acid are excreted into breast milk; use with caution in nursing mothers, monitoring the infant for diarrhea, candidiasis, and hypersensitivity reactions.
