Overview of the medicine
Staxiclav 500 mg tablet is a broad-spectrum antibiotic containing Amoxicillin and Clavulanic Acid. It is used to treat a wide range of bacterial infections, including respiratory tract infections, urinary tract infections, skin and soft tissue infections, and dental infections.
Uses & Indications
Dosage
Adults
One 500 mg/125 mg tablet every 8 hours, or as directed by physician. For more severe infections, dosage may be increased.
Elderly
No specific dose adjustment is required unless there is severe renal impairment. Monitor renal function.
Renal_impairment
Adjust dose based on creatinine clearance. For CrCl 10-30 mL/min: 500 mg/125 mg every 12 hours. For CrCl <10 mL/min: 500 mg/125 mg every 24 hours.
How to Take
Take orally at the start of a meal to minimize potential gastrointestinal intolerance and enhance absorption of clavulanic acid. Swallow the tablet whole with water.
Mechanism of Action
Amoxicillin inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis. Clavulanic acid is a beta-lactamase inhibitor that irreversibly binds to and inactivates beta-lactamase enzymes produced by bacteria, thus protecting amoxicillin from degradation.

Excretion
Primarily eliminated unchanged by the kidneys (glomerular filtration and tubular secretion) for amoxicillin. Clavulanic acid is excreted renally and non-renally.
Half life
Approximately 1 to 1.5 hours for both amoxicillin and clavulanic acid.
Absorption
Both amoxicillin and clavulanic acid are well absorbed from the gastrointestinal tract after oral administration. Peak plasma concentrations are reached in approximately 1 to 2 hours.
Metabolism
Amoxicillin is minimally metabolized. Clavulanic acid is extensively metabolized.
Side Effects
Contraindications
- History of hypersensitivity to penicillin or any other beta-lactam antibiotics
- History of cholestatic jaundice or hepatic dysfunction associated with amoxicillin/clavulanic acid therapy
- Severe renal impairment (CrCl <10 mL/min) without dose adjustment
Drug Interactions
Probenecid
May increase and prolong plasma levels of amoxicillin by decreasing its renal tubular secretion. Concomitant use is not recommended.
Allopurinol
Concomitant use may increase the incidence of skin rashes.
Methotrexate
Amoxicillin may reduce methotrexate excretion, leading to increased toxicity. Monitor methotrexate levels.
Oral contraceptives
May reduce the efficacy of oral contraceptives. Advise alternative or additional contraceptive methods.
Oral anticoagulants (e.g., Warfarin)
May prolong prothrombin time; careful monitoring is necessary.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include gastrointestinal upset (nausea, vomiting, diarrhea), fluid and electrolyte imbalance, and in rare cases, crystalluria leading to renal failure. Treatment is symptomatic and supportive. Hemodialysis can be used to remove amoxicillin and clavulanic acid from circulation.
Pregnancy & Lactation
Pregnancy Category B. Use during pregnancy only if clearly needed. Both amoxicillin and clavulanic acid are excreted into breast milk in small amounts; use with caution in breastfeeding mothers. Monitor infants for diarrhea, candidiasis, and skin rash.
