Description
Overview of the medicine
Steclo-005 Ointment contains Clobetasol Propionate, a highly potent corticosteroid, used for short-term treatment of severe inflammatory skin conditions that are unresponsive to less potent corticosteroids. It helps reduce inflammation, itching, and redness.
Uses & Indications
Dosage
Adults
Apply a thin film to the affected skin areas once or twice daily. Treatment should not exceed two consecutive weeks, and the total amount should not exceed 50g per week.
Elderly
Similar to adult dosage, but use with caution and monitor for skin thinning and systemic effects.
Renal_impairment
No specific dose adjustment needed for topical application, but caution with extensive use due to potential systemic absorption.
How to Take
For topical use only. Apply a thin layer to the affected skin and gently rub in. Do not apply to the face, groin, or axillae unless directed by a doctor. Wash hands before and after application. Do not use occlusive dressings unless specifically advised by a physician.
Mechanism of Action
Clobetasol propionate is a corticosteroid that primarily acts as an anti-inflammatory and immunosuppressive agent. It works by inducing phospholipase A2 inhibitory proteins, called lipocortins, which control the biosynthesis of potent mediators of inflammation such as prostaglandins and leukotrienes.
Pharmacokinetics
Onset
Relief of symptoms can begin within hours to days.
Excretion
Excreted mainly in urine and feces as metabolites.
Half life
Not well-defined for topical application; systemic half-life of absorbed drug is typically several hours (e.g., 3-6 hours).
Absorption
Systemic absorption varies with vehicle, skin integrity, and application site; generally low but increases with occlusion or inflamed skin. Peak plasma levels are usually very low (nanograms/mL).
Metabolism
Primarily metabolized in the liver to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Clobetasol Propionate or any component
- Rosacea
- Acne vulgaris
- Perioral dermatitis
- Pruritus without inflammation
- Viral, fungal, or bacterial skin infections (untreated)
Drug Interactions
Other Topical Corticosteroids
Avoid concomitant use to prevent additive systemic effects and local adverse reactions.
CYP3A4 Inhibitors (e.g., Ritonavir, Itraconazole)
May increase systemic exposure to Clobetasol, leading to potential for HPA axis suppression. Use with caution.
Storage
Store below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Acute overdose is unlikely. Chronic overdose or misuse can lead to signs of hypercorticism (e.g., Cushing's syndrome) and HPA axis suppression. Treatment is symptomatic and supportive. Gradual withdrawal may be necessary.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Use with caution in nursing mothers; systemic absorption is minimal, but risks to infant should be weighed against maternal benefit.

YesClinical Trials
Extensive clinical trials have established the efficacy and safety of Clobetasol Propionate for its approved indications. Studies typically involve randomized, double-blind, placebo-controlled trials demonstrating superiority over placebo and non-inferiority to other potent corticosteroids.
Lab Monitoring
- No routine lab monitoring for short-term topical use. For extensive or prolonged use, consider monitoring for HPA axis suppression (e.g., morning plasma cortisol levels, ACTH stimulation test).
Doctor Notes
- High potency steroid, limit duration of use.
- Educate patients on proper application technique and potential side effects, especially skin thinning.
- Consider step-down therapy to less potent steroids for maintenance if prolonged treatment is needed.
Patient Guidelines
- Use only as directed by your doctor.
