Overview of the medicine
Strazyl 400 mg tablet contains Linezolid, an oxazolidinone antibiotic used to treat serious bacterial infections that are resistant to other antibiotics. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
600 mg every 12 hours for 10-14 consecutive days for most infections (e.g., VRE, nosocomial pneumonia). For complicated skin and skin structure infections, 400 mg or 600 mg every 12 hours.
Elderly
No dose adjustment necessary.
Renal_impairment
No dose adjustment needed for mild to severe renal impairment. Hemodialysis removes Linezolid; administer dose after dialysis.
How to Take
Strazyl 400 mg tablets can be taken with or without food. Swallow the tablet whole with a glass of water. Take it at regular intervals as prescribed by your doctor.
Mechanism of Action
Linezolid exerts its antibacterial activity by inhibiting bacterial protein synthesis. It binds to the 23S ribosomal RNA of the 50S ribosomal subunit, preventing the formation of a functional 70S initiation complex, which is essential for bacterial translation.

Rapid, therapeutic levels achieved within hours.
Excretion
Approximately 30% excreted in urine as unchanged drug, and about 50% as metabolites. A smaller portion excreted in feces.
Half life
Approximately 4-5 hours.
Absorption
Rapidly and extensively absorbed after oral administration; absolute bioavailability is approximately 100%. Peak plasma concentrations are reached in 1-2 hours.
Metabolism
Primarily metabolized by oxidation of the morpholine ring, resulting in two inactive carboxylic acid metabolites. Not metabolized by cytochrome P450 enzymes.

Contraindications
- Hypersensitivity to Linezolid or any component of the formulation
- Patients taking MAO inhibitors or within two weeks of taking an MAO inhibitor
- Uncontrolled hypertension
- Pheochromocytoma
- Carcinoid syndrome
- Thyrotoxicosis
- Untreated hyperthyroidism
- Patients taking other serotonergic drugs unless carefully monitored

Risk of serotonin syndrome. Monitor for symptoms.
Tyramine-rich foods/drinks
Potential for hypertensive crisis due to weak MAO inhibition. Avoid large amounts.

Potentiation of vasopressor effects. Monitor blood pressure.
Monoamine Oxidase Inhibitors (MAOIs)
Linezolid is a reversible, non-selective MAOI. Concomitant use with other MAOIs is contraindicated.

Store at room temperature, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptomatic and supportive measures should be taken. Hemodialysis may remove Linezolid, but it is not sufficient for rapid removal.

Pregnancy: Category C. Use only if potential benefit justifies the potential risk to the fetus. Lactation: Excreted in breast milk. A decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
