Description
Overview of the medicine
Sucrasol (Sucralfate) is a cytoprotective agent used for the short-term treatment and prevention of ulcers in the stomach and duodenum. It works by forming a protective barrier over the ulcer surface, shielding it from acid and enzymes.
Uses & Indications
Dosage
Adults
For active duodenal ulcer: 1 gram four times daily (one hour before meals and at bedtime) or 2 grams twice daily (morning and bedtime) for 4-8 weeks. For maintenance: 1 gram twice daily.
Elderly
No specific dose adjustment required, but caution with renal impairment due to aluminum content.
Renal_impairment
Use with caution due to aluminum accumulation risk. May require dose reduction or alternative therapy in severe renal impairment.
How to Take
Take on an empty stomach, usually one hour before meals and at bedtime. Do not chew or crush tablets; swallow whole with water. Shake suspension well before use.
Mechanism of Action
Sucralfate adheres to ulcer sites, forming a viscous, adhesive gel that creates a protective barrier against acid, pepsin, and bile salts. It also stimulates prostaglandin synthesis, mucus, and bicarbonate secretion, contributing to mucosal protection and healing.
Pharmacokinetics
Onset
Within 30 minutes for protective action.
Excretion
Primarily excreted unchanged in feces. The small absorbed portion is excreted in urine.
Half life
Not applicable due to minimal systemic absorption; the small absorbed portion is rapidly eliminated.
Absorption
Minimally absorbed from the gastrointestinal tract (less than 5% of the dose is absorbed).
Metabolism
Not metabolized.
Side Effects
Contraindications
- •Hypersensitivity to sucralfate or any component of the formulation.
- •Patients with severe chronic renal failure (due to potential for aluminum accumulation and toxicity).
Drug Interactions
Antacids
Concurrent use may reduce the binding capacity of sucralfate. Separate administration by at least 30 minutes.
H2-receptor antagonists (e.g., cimetidine) and PPIs (e.g., omeprazole)
These drugs may reduce gastric acid, which can interfere with sucralfate activation. Administer sucralfate at least 30 minutes before or 2 hours after these drugs.
Warfarin, Phenytoin, Digoxin, Ketoconazole, Fluoroquinolones (e.g., ciprofloxacin)
Sucralfate can decrease the absorption of these drugs. Administer these drugs at least 2 hours before sucralfate.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Overdose is rare due to minimal systemic absorption. Symptoms may include nausea, vomiting, indigestion, abdominal pain. Management is supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown a risk, but human studies are lacking. Generally considered safe for use during pregnancy. Excretion into breast milk is unknown, but systemic absorption is minimal, so risk to infant is low. Consult doctor.
