Description
Overview of the medicine
Aceclofenac is an NSAID used for pain and inflammation associated with musculoskeletal disorders. It works by inhibiting prostaglandin synthesis. The 200mg extended-release formulation provides prolonged therapeutic effect.
Uses & Indications
Dosage
Adults
200 mg once daily (extended-release tablet).
Elderly
Same as adults, but caution is advised due to increased risk of side effects. Lower effective dose may be considered.
Renal_impairment
Initial dose adjustment may be required; use with caution. Not recommended in severe renal impairment.
How to Take
Take the tablet whole with water, preferably with or after food to minimize gastrointestinal upset. Do not crush or chew the extended-release tablet.
Mechanism of Action
Aceclofenac works primarily by inhibiting the enzyme cyclo-oxygenase (COX), which is involved in the synthesis of prostaglandins. Prostaglandins are mediators of pain, inflammation, and fever. It preferentially inhibits COX-2, leading to anti-inflammatory, analgesic, and antipyretic effects.
Pharmacokinetics
Onset
Within 1-2 hours for pain relief, full anti-inflammatory effect may take longer.
Excretion
Approximately two-thirds of the dose is excreted in the urine, mainly as hydroxylated metabolites, and one-third in the faeces.
Half life
Approximately 4-4.3 hours for immediate release; effective half-life for ER is prolonged due to sustained release.
Absorption
Well absorbed after oral administration, peak plasma concentrations typically reached in 2-3 hours for immediate release, but prolonged for ER. Bioavailability is nearly 100%.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C9, into 4'-hydroxyaceclofenac and other metabolites.

- Hypersensitivity to aceclofenac or other NSAIDs (e.g., aspirin-induced asthma, urticaria)
- Active peptic ulcer or gastrointestinal bleeding
- Severe renal or hepatic impairment
- Severe heart failure
- Third trimester of pregnancy

Methotrexate
Increased methotrexate toxicity.
Lithium, Digoxin
Increased plasma levels of lithium/digoxin.
Corticosteroids, SSRIs
Increased risk of GI bleeding.
Diuretics, ACE inhibitors
Reduced antihypertensive effect, increased risk of renal impairment.
Warfarin, other anticoagulants
Increased risk of bleeding.

Overdose
Symptoms may include headache, nausea, vomiting, epigastric pain, GI bleeding, disorientation, excitation, coma, drowsiness, dizziness, tinnitus, and convulsions. Management includes gastric lavage, activated charcoal, and supportive symptomatic treatment.
Pregnancy & Lactation
Not recommended during the third trimester of pregnancy due to potential for premature closure of the fetal ductus arteriosus. Use during other trimesters or lactation only if potential benefits outweigh risks, and under strict medical supervision.
