Description
Overview of the medicine
Sugatrol (Sugliflozin) is an SGLT2 inhibitor used to manage type 2 diabetes mellitus. It helps to lower blood glucose by increasing glucose excretion in the urine.
Uses & Indications
Dosage
Adults
The recommended starting dose is 50 mg once daily in the morning, with or without food. The dose may be increased up to 100 mg once daily based on glycemic response and tolerability.
Elderly
No dose adjustment is required based solely on age. However, renal function should be monitored due to potential age-related decline.
Renal_impairment
For eGFR 30 to <60 mL/min/1.73m², the dose should not exceed 50 mg daily. Not recommended for patients with eGFR <30 mL/min/1.73m² or those on dialysis.
How to Take
Sugatrol tablets should be taken orally once daily in the morning, with or without food. Swallow the tablet whole with water.
Mechanism of Action
Sugliflozin works by inhibiting Sodium-Glucose Cotransporter 2 (SGLT2) in the renal tubules, which is responsible for reabsorbing glucose from the glomerular filtrate back into the blood. By inhibiting SGLT2, sugliflozin reduces glucose reabsorption and increases urinary glucose excretion, thereby lowering blood glucose levels.
Pharmacokinetics
Onset
Glycosuric effect observed within hours of administration.
Excretion
Approximately 70% excreted via urine (as metabolites) and 30% via feces.
Half life
Approximately 10-15 hours, allowing for once-daily dosing.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations reached within 1-2 hours. High oral bioavailability.
Metabolism
Primarily metabolized via O-glucuronidation by UGT1A9 and UGT2B7.
Side Effects
Contraindications
- Hypersensitivity to sugliflozin or any excipients.
- Patients with severe renal impairment (eGFR <30 mL/min/1.73m²), end-stage renal disease (ESRD), or on dialysis.
Drug Interactions
Diuretics
May increase the risk of dehydration and hypotension, especially in elderly patients or those with renal impairment.
ACE Inhibitors/ARBs
May enhance hypotensive effect when used with blood pressure lowering agents.
Insulin and Insulin Secretagogues (e.g., Sulfonylureas)
Increased risk of hypoglycemia when used concomitantly. A lower dose of insulin or sulfonylurea may be required.
Storage
Store below 30°C (86°F) in a dry place. Protect from light and moisture. Do not freeze.
Overdose
In the event of an overdose, initiate appropriate supportive treatment based on the patient's clinical presentation. Sugliflozin is unlikely to be dialyzable.
Pregnancy & Lactation
Not recommended during pregnancy, especially during the second and third trimesters, due to potential adverse effects on fetal renal development. It is unknown if sugliflozin is excreted in human milk; therefore, breastfeeding is not recommended.

WHO Essential Medicine
YesClinical Trials
Clinical trials for sugliflozin demonstrated significant reductions in HbA1c, fasting plasma glucose, and body weight in patients with type 2 diabetes. Large-scale cardiovascular outcome trials also showed a reduction in the risk of major adverse cardiovascular events and improvement in renal outcomes.
Lab Monitoring
- Renal function (eGFR) should be assessed prior to initiation and periodically thereafter.
- Blood glucose levels and HbA1c should be monitored regularly.
- Lipid profile may be monitored.
- Monitor for signs and symptoms of DKA.
Doctor Notes
- Emphasize the importance of consistent renal function monitoring before and during Sugatrol treatment.
- Educate patients about the signs and symptoms of DKA and genital/urinary tract infections to ensure timely intervention.
- Consider Sugatrol as a first-line agent or add-on therapy for patients with type 2 diabetes, especially those with established cardiovascular disease, heart failure with reduced ejection fraction, or chronic kidney disease, due to its proven cardiorenal benefits.
Patient Guidelines
- Stay well-hydrated, especially during illness or in hot weather.
- Monitor blood glucose levels regularly as advised by your doctor.
- Be aware of and report symptoms of genital yeast infections or urinary tract infections.
- Recognize symptoms of DKA (nausea, vomiting, abdominal pain, excessive thirst, rapid breathing) and seek immediate medical attention if they occur.
- Inform your doctor about all medications you are taking, including over-the-counter drugs and herbal supplements.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Sugatrol usually has no or negligible influence on the ability to drive and use machines. However, patients should be advised to take precautions to avoid hypoglycemia, especially when used in combination with insulin or sulfonylureas, which can lead to dizziness.
Lifestyle Advice
- Maintain a balanced, healthy diet and follow any dietary recommendations from your healthcare provider.
- Engage in regular physical activity as appropriate for your health condition.
- Ensure adequate fluid intake to prevent dehydration, especially when taking Sugatrol.
