Description
Overview of the medicine
Sulfacol is a combination antibiotic containing sulfamethoxazole and trimethoprim, used to treat a wide range of bacterial infections including urinary tract infections, respiratory tract infections, and certain types of diarrhea.
Uses & Indications
Dosage
Adults
Adults: 800 mg Sulfamethoxazole + 160 mg Trimethoprim (DS Tablet) orally every 12 hours for 5 to 14 days, depending on the infection.
Elderly
Elderly patients may require dosage adjustment based on renal function.
Renal_impairment
Patients with significant renal impairment require dose reduction or are contraindicated.
How to Take
Take orally with a glass of water, with or without food. Taking with food or milk may reduce stomach upset. Drink plenty of fluids to prevent crystalluria.
Mechanism of Action
Sulfamethoxazole inhibits bacterial dihydrofolate synthesis, and trimethoprim inhibits bacterial dihydrofolate reductase, leading to sequential blockade of the bacterial folic acid pathway and bactericidal action.

Effects typically begin within 1-4 hours.
Excretion
Excreted mainly via the kidneys.
Half life
Sulfamethoxazole: 10-12 hours; Trimethoprim: 8-10 hours.

Side Effects
Contraindications

Drug Interactions
Warfarin
May enhance anticoagulant effect, increasing risk of bleeding.
Phenytoin
May inhibit phenytoin metabolism, leading to increased phenytoin levels.
Cyclosporine
May result in increased nephrotoxicity and decreased cyclosporine levels.
Methotrexate
Increased methotrexate levels and toxicity due to reduced renal excretion.
Diuretics (especially thiazides)
Increased risk of thrombocytopenia with purpura in elderly patients.
Storage
Store below 30°C, in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of acute overdose include nausea, vomiting, dizziness, and mental confusion. Chronic overdose can lead to bone marrow depression. Treatment is symptomatic and supportive; gastric lavage, forced diuresis, and hemodialysis may be considered.
Pregnancy & Lactation
Pregnancy Category C (D if used in late pregnancy). Contraindicated in late pregnancy due to risk of kernicterus in the neonate. Excreted in breast milk; use with caution during lactation, monitor infant for rash and diarrhea.
