Description
Overview of the medicine
Sulforid 2 mg Tablet contains Glimepiride, an oral antidiabetic medication used to lower blood glucose levels in patients with type 2 diabetes mellitus when diet and exercise alone are not sufficient.
Uses & Indications
Dosage
Adults
Initial dose 1-2 mg once daily, taken with breakfast or first main meal. Max 8 mg/day.
Elderly
Start with a lower dose (e.g., 1 mg once daily) and titrate carefully.
Renal_impairment
Initial dose of 1 mg once daily, titrate carefully. Not recommended for severe renal impairment.
How to Take
Take orally once daily with breakfast or the first main meal. Swallow the tablet whole with water.
Mechanism of Action
Glimepiride stimulates insulin release from pancreatic beta cells, primarily by closing ATP-sensitive potassium channels on the cell membrane, leading to depolarization and calcium influx, which triggers insulin secretion.
Pharmacokinetics
Onset
2-3 hours
Excretion
Approximately 60% excreted in urine and 40% in feces, mainly as metabolites.
Half life
5-8 hours
Absorption
Rapidly and completely absorbed from the gastrointestinal tract, with peak plasma concentrations occurring within 2-3 hours.
Metabolism
Completely metabolized in the liver by cytochrome P450 2C9 (CYP2C9) to two main metabolites.
Side Effects
Contraindications
- Hypersensitivity to glimepiride or other sulfonylureas or sulfonamides.
- Type 1 Diabetes Mellitus.
- Diabetic ketoacidosis.
- Severe renal or hepatic impairment.
- Pregnancy and lactation.
Drug Interactions
Warfarin
May potentiate or diminish the anticoagulant effect.
Rifampicin
May decrease glimepiride levels, potentially causing hyperglycemia.
Fluconazole, cimetidine, allopurinol
May increase glimepiride levels, potentially causing hypoglycemia.
Beta-blockers, ACE inhibitors, MAO inhibitors
May enhance the glucose-lowering effect, increasing risk of hypoglycemia.
Thiazide diuretics, corticosteroids, sympathomimetics
May reduce the glucose-lowering effect, increasing risk of hyperglycemia.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms include severe hypoglycemia (e.g., sweating, tremor, hunger, confusion, coma). Management involves immediate administration of glucose (oral or IV), monitoring blood glucose, and supportive care.
Pregnancy & Lactation
Contraindicated during pregnancy and lactation. Insulin is generally preferred for blood glucose control during pregnancy. Consult a doctor immediately if pregnant or breastfeeding.
