Overview of the medicine
Superpime 1 gm Injection contains Cefepime, a broad-spectrum, fourth-generation cephalosporin antibiotic used to treat a variety of serious bacterial infections, including pneumonia, urinary tract infections, complicated intra-abdominal infections, and febrile neutropenia.
Uses & Indications
Dosage
Adults
Typically 1-2 gm IV/IM every 8-12 hours, depending on the severity and type of infection. For febrile neutropenia, 2 gm IV every 8 hours.
Elderly
Dosage adjustment may be necessary based on renal function.
Renal_impairment
Significant dosage reduction is required based on creatinine clearance. For example, for creatinine clearance 30-60 mL/min, 1 gm every 24 hours; for 11-29 mL/min, 0.5 gm every 24 hours.
How to Take
Administered intravenously (IV) over 30 minutes or intramuscularly (IM) into a large muscle mass. The powder must be reconstituted with an appropriate diluent (e.g., sterile water for injection, 0.9% sodium chloride).
Mechanism of Action
Cefepime acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located in the bacterial cell wall, which prevents the cross-linking of peptidoglycan units, leading to cell lysis and bacterial death.

Primarily excreted by the kidneys (approximately 85% as unchanged drug in urine).
Half life
Approximately 2 hours in adults with normal renal function.
Absorption
Rapid and complete absorption after intramuscular (IM) or intravenous (IV) administration. Peak plasma concentrations are achieved rapidly after IV infusion.
Metabolism
Not extensively metabolized. A small fraction is hydrolyzed to N-methylpyrrolidine, which is then rapidly converted to N-methylpyrrolidine N-oxide.

Contraindications
- Hypersensitivity to cefepime or any component of the formulation.
- Hypersensitivity to other cephalosporin antibiotics.
- History of severe hypersensitivity reactions to any other beta-lactam antibacterial agents (e.g., penicillins, carbapenems).
Drug Interactions
Aminoglycosides
Concurrent use may increase the risk of nephrotoxicity and ototoxicity. Monitor renal function closely.
Loop Diuretics (e.g., Furosemide)
May increase the risk of nephrotoxicity when co-administered with cephalosporins.
Oral Anticoagulants (e.g., Warfarin)
Cephalosporins may enhance the anticoagulant effect; monitor INR/PT time.
