Description
Overview of the medicine
Superpime 500 mg Injection contains Cefepime, a fourth-generation cephalosporin antibiotic used to treat various serious bacterial infections, including pneumonia, urinary tract infections, skin infections, and febrile neutropenia.
Uses & Indications
Dosage
Adults
Typical dose is 1-2 g administered intravenously (IV) or intramuscularly (IM) every 8-12 hours, depending on the severity and type of infection. For febrile neutropenia, 2 g IV every 8 hours.
Elderly
Dosage should be adjusted based on renal function and creatinine clearance. Lower doses or extended dosing intervals may be necessary.
Renal_impairment
Significant dosage reduction is required. Initial dose may be standard, but subsequent doses and frequency must be adjusted based on creatinine clearance (e.g., for CrCl 30-60 mL/min, 0.5-1 g every 24 hours).
How to Take
Superpime 500 mg Injection is administered via slow intravenous infusion (over 30 minutes) or deep intramuscular injection. For IV administration, the powder should be reconstituted with an appropriate diluent (e.g., Sterile Water for Injection, 5% Dextrose Injection) and then diluted further before infusion. For IM administration, reconstitute with Sterile Water for Injection or 0.5% Lidocaine HCl Solution.
Mechanism of Action
Cefepime acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located in the bacterial cell wall, which interferes with the transpeptidation step of peptidoglycan synthesis, leading to cell wall defects and bacterial cell lysis.
Pharmacokinetics
Onset
Rapid (within minutes after IV administration).
Excretion
Primarily renal excretion (approximately 85% of an administered dose is excreted unchanged in the urine by glomerular filtration).
Half life
Approximately 2 hours in individuals with normal renal function.
Absorption
Rapidly and completely absorbed after intramuscular (IM) or intravenous (IV) administration. Peak plasma concentrations are achieved rapidly.
Metabolism
Minimally metabolized; less than 1% is metabolized to an inactive N-methylpyrrolidine metabolite.
Side Effects
Contraindications
- Known hypersensitivity to cefepime or any other cephalosporin antibiotics.
- Known hypersensitivity to penicillin or other beta-lactam antibiotics (due to potential cross-reactivity).
- History of severe immediate hypersensitivity reactions to beta-lactam antibiotics.
Drug Interactions
Aminoglycosides
Increased risk of nephrotoxicity and ototoxicity when co-administered with cefepime. Close monitoring of renal function is recommended.
Loop Diuretics (e.g., Furosemide)
May increase the serum concentrations of cefepime, especially in patients with impaired renal function, potentially increasing the risk of adverse effects.
Oral Anticoagulants (e.g., Warfarin)
Cephalosporins, including cefepime, may affect vitamin K synthesis and coagulation, potentially enhancing the anticoagulant effect. INR monitoring is advised.
Storage
Store unreconstituted vials at controlled room temperature (20-25°C) and protect from light. After reconstitution, refer to specific manufacturer instructions for storage duration and temperature (usually stable for 24 hours at room temperature or up to 7 days under refrigeration).
Overdose
Symptoms of overdose may include encephalopathy (impaired consciousness, confusion, hallucinations, stupor, coma), myoclonus, and seizures. Management should be supportive, including discontinuation of the drug. Hemodialysis may be useful in facilitating the removal of cefepime from the body, especially in patients with impaired renal function.
Pregnancy & Lactation
Pregnancy Category B. Animal reproduction studies have not demonstrated a risk to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Cefepime is excreted in human breast milk in small quantities; caution should be exercised when administered to a nursing woman.

YesClinical Trials
Cefepime has undergone numerous clinical trials demonstrating its efficacy and safety in various severe bacterial infections across different patient populations, including those with febrile neutropenia and complicated intra-abdominal infections.
Lab Monitoring
- Renal function tests (serum creatinine, BUN) should be closely monitored, especially in patients with pre-existing renal impairment or those receiving concomitant nephrotoxic drugs.
- Complete blood count (CBC) with differential, especially during prolonged therapy.
- Liver function tests (ALT, AST, alkaline phosphatase, bilirubin) intermittently.
- Coagulation parameters (INR/PT/aPTT) if there is a risk of bleeding or concomitant anticoagulant use.
Doctor Notes
- Strict adherence to dosage adjustment guidelines for renal impairment is critical to prevent neurotoxicity, which can manifest as altered mental status, myoclonus, and seizures.
- Consider combination therapy for polymicrobial infections, particularly for complicated intra-abdominal infections where anaerobic coverage is needed (e.g., with metronidazole).
- Monitor for signs of C. difficile-associated diarrhea (CDAD) during and after treatment, especially in patients presenting with severe, persistent diarrhea.
Patient Guidelines
- It is crucial to complete the entire course of Superpime Injection as prescribed by your doctor, even if you start feeling better. Stopping early can lead to the return of the infection and development of antibiotic resistance.
- Report any severe or persistent side effects, such as severe diarrhea, rash, or neurological symptoms (confusion, seizures) to your doctor immediately.
- Do not share this medicine with others, even if they have similar symptoms, as it may not be appropriate for their condition.
Missed Dose Advice
If a dose is missed, administer it as soon as you remember, unless it is almost time for the next scheduled dose. In that case, skip the missed dose and continue with the regular dosing schedule. Do not administer a double dose to make up for a missed one.
Driving Precautions
While rare, cefepime can cause neurological side effects such as dizziness, confusion, or seizures. Patients should be cautious when driving or operating machinery until they know how the medication affects them.
Lifestyle Advice
- Stay well-hydrated by drinking plenty of fluids during the course of treatment.
- Maintain good hygiene to prevent further infections.
