Description
Overview of the medicine
Suxen (Naproxen) is a non-steroidal anti-inflammatory drug (NSAID) used to relieve pain, fever, inflammation, and stiffness. It works by inhibiting prostaglandin synthesis.
Uses & Indications
Dosage
Adults
250-500 mg twice daily. Maximum 1000 mg/day for chronic conditions. For acute pain, initial 500 mg, then 250 mg every 6-8 hours as needed.
Elderly
Lower doses may be considered due to increased risk of adverse effects.
Renal_impairment
Not recommended for severe renal impairment (creatinine clearance < 30 mL/min). Lower doses and careful monitoring for moderate impairment.
How to Take
Take with food or milk to minimize gastrointestinal upset. Swallow whole with water, do not crush or chew.
Mechanism of Action
Naproxen non-selectively inhibits both cyclooxygenase-1 (COX-1) and cyclooxygenase-2 (COX-2) enzymes, which are responsible for the synthesis of prostaglandins from arachidonic acid. Prostaglandins are mediators of pain, inflammation, and fever.
Pharmacokinetics
Onset
Analgesic effect within 1 hour.
Excretion
Eliminated primarily in the urine (approximately 95%) as naproxen, 6-O-desmethylnaproxen, and their conjugates. A small amount is excreted in feces.
Half life
12-17 hours.
Absorption
Rapidly and completely absorbed from the gastrointestinal tract. Peak plasma concentrations reached within 2-4 hours.
Metabolism
Primarily metabolized in the liver by the cytochrome P450 system to 6-O-desmethylnaproxen, which is then glucuronidated.
Side Effects
Contraindications
- •History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs.
- •Active peptic ulcer, gastrointestinal bleeding, severe hepatic or renal impairment, severe heart failure.
Drug Interactions
Lithium
Increased plasma lithium levels.
Warfarin
Increased risk of bleeding.
ACE inhibitors/ARBs
Reduced antihypertensive effect, increased risk of renal impairment.
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children.
Overdose
Symptoms include drowsiness, heartburn, nausea, vomiting, epigastric pain. Management includes symptomatic and supportive care. Gastric lavage and activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy Category C (first and second trimesters), Category D (third trimester). Avoid in late pregnancy due to potential premature closure of fetal ductus arteriosus. Not recommended during breastfeeding as it is excreted in breast milk.
