Description
Overview of the medicine
Syndol Plus is a combination medicine containing paracetamol, codeine phosphate, and caffeine. It is used for the short-term relief of acute moderate pain that is not relieved by paracetamol, ibuprofen, or aspirin alone. It is often prescribed for tension headaches, migraines, muscle pain, and period pain.
Uses & Indications
Dosage
Adults
1-2 tablets every 4-6 hours as needed. Do not exceed 8 tablets in 24 hours.
Elderly
Reduced dosage may be required, consult a physician.

How to Take
Take orally with water, with or without food.
Mechanism of Action
Paracetamol works by inhibiting prostaglandin synthesis in the central nervous system. Codeine is an opioid analgesic that acts on mu-opioid receptors in the brain and spinal cord, reducing the perception of pain. Caffeine is a CNS stimulant that enhances the analgesic effects of paracetamol and codeine.

Absorption
All components are well absorbed orally. Paracetamol peak plasma concentrations are reached within 30-60 minutes, codeine within 60 minutes, and caffeine within 30-60 minutes.
Metabolism
Paracetamol is primarily metabolized in the liver (glucuronidation, sulfation, and oxidation). Codeine is metabolized in the liver by CYP2D6 to morphine (active) and by CYP3A4 to norcodeine. Caffeine is extensively metabolized in the liver by CYP1A2.

Contraindications
- •Hypersensitivity to any components, acute respiratory depression, paralytic ileus, severe liver disease, acute alcoholism, head injury, opioid dependence, children under 12 years (due to codeine).
- •Severe asthma.
Drug Interactions
Alcohol
Increased CNS depression and risk of liver damage.
MAO Inhibitors
Risk of serotonin syndrome with codeine.
Anticoagulants (e.g., Warfarin)
May enhance anticoagulant effect (Paracetamol).
Certain antidepressants (e.g., SSRIs, tricyclics)
May alter codeine metabolism and increase risk of side effects.
Other CNS depressants (e.g., benzodiazepines, other opioids)

Overdose
Overdose symptoms include severe drowsiness, shallow breathing, slow heartbeat, nausea, vomiting, abdominal pain (due to paracetamol), and liver damage. Seek immediate medical attention. Treatment involves supportive care and specific antidotes like naloxone for opioid toxicity and N-acetylcysteine for paracetamol toxicity.
Pregnancy & Lactation
Use during pregnancy is generally not recommended, especially in the third trimester due to risk of neonatal opioid withdrawal syndrome. Small amounts of codeine and its metabolites are excreted in breast milk; use with caution or avoid during lactation. Consult a doctor.
