Description
Overview of the medicine
Tinidazole is an antiprotozoal and antibacterial medication used to treat various infections caused by susceptible organisms, including amoebiasis, giardiasis, trichomoniasis, and certain anaerobic bacterial infections.
Uses & Indications
Dosage
Adults
Dosage varies with infection type. For amoebiasis: 2 g once daily for 2-3 days or 500 mg twice daily for 5-7 days. For trichomoniasis/giardiasis: single 2 g dose.
Elderly
No specific dosage adjustment required, but consider overall health and renal function.
Renal_impairment
Dose adjustment may be necessary in patients with severe renal impairment (creatinine clearance < 22 ml/min).
How to Take
Take orally, preferably with or after food, to minimize gastrointestinal discomfort. Swallow the tablet whole with water.
Mechanism of Action
Tinidazole exerts its effects by entering the cell of anaerobic and protozoal organisms. Once inside, it is reduced to active metabolites that interact with DNA, causing DNA strand breakage and inhibition of nucleic acid synthesis, leading to cell death.
Pharmacokinetics
Onset
Within 1-2 hours for therapeutic effect.
Excretion
Excreted primarily through the kidneys (unchanged drug and metabolites) and also via faeces.
Half life
Approximately 12-14 hours, allowing for once-daily dosing in many cases.
Absorption
Rapidly and almost completely absorbed after oral administration. Peak plasma concentrations are reached within 2 hours.
Metabolism
Partially metabolized in the liver, primarily by oxidation.
Side Effects
Contraindications
- Hypersensitivity to tinidazole or other nitroimidazole derivatives.
- First trimester of pregnancy.
- Patients with active neurological disorders or a history of blood dyscrasias.
Drug Interactions
Alcohol
Disulfiram-like reaction (abdominal cramps, nausea, vomiting, flushing, headaches) if consumed during or within 72 hours of treatment.
Lithium
May increase serum lithium levels, leading to toxicity.
Oral Anticoagulants (e.g., Warfarin)
May potentiate the anticoagulant effect, requiring dose adjustment and increased monitoring.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Overdose symptoms may include severe gastrointestinal upset, ataxia, and neurological symptoms. Management involves symptomatic and supportive care. No specific antidote is available. Hemodialysis may be helpful.
Pregnancy & Lactation
Contraindicated in the first trimester of pregnancy. Use in the second and third trimesters only if clearly needed and potential benefits outweigh risks. Not recommended during lactation; mothers should discontinue breastfeeding if treatment is essential.
