Description
Overview of the medicine
Tamicol (Colistin Sulphate) is a polymyxin antibiotic used to treat severe infections caused by susceptible Gram-negative bacteria, particularly those resistant to other antibiotics. It is often used for gastrointestinal infections in pediatric patients.
Uses & Indications
Dosage
Adults
For gastrointestinal infections: 1.5-3 million IU (equivalent to 75-150 mg colistin sulphate) orally daily in 2-3 divided doses. This can be varied based on severity and physician's advice.
Elderly
Similar to adult dosage, no specific dose adjustment needed due to minimal systemic absorption when used orally.
Renal_impairment
No dose adjustment required for renal impairment when administered orally for local gut infections, as systemic absorption is minimal.
How to Take
For oral use only. Reconstitute the dry powder with boiled and cooled water as per instructions on the bottle. Shake well before each use.
Mechanism of Action
Colistin acts as a cationic detergent, disrupting the outer and cytoplasmic membranes of Gram-negative bacteria by interacting with phospholipids, leading to increased permeability and leakage of intracellular components, resulting in bacterial cell death.
Pharmacokinetics
Onset
Within hours for local gut action, clinical improvement seen within 24-48 hours.
Excretion
Mainly excreted unchanged in feces following oral administration.
Half life
Systemic half-life is approximately 2-3 hours, but for oral administration, most acts locally in the gut.
Absorption
Oral colistin sulphate is minimally absorbed from the intact gastrointestinal tract, making it suitable for local gut infections. Systemic absorption is negligible.
Metabolism
Minimal metabolism occurs.
Side Effects
Contraindications
- •Hypersensitivity to colistin or other polymyxins
- •Severe renal impairment (for systemic use, less relevant for oral)
Drug Interactions
Nephrotoxic drugs
Concomitant use with other nephrotoxic drugs (e.g., aminoglycosides) should be avoided or closely monitored if systemic absorption occurs, but this is rare with oral colistin.
Neuromuscular blockers
Concurrent use with neuromuscular blocking agents may enhance muscle weakness, though systemic absorption is minimal with oral use.
Storage
Store dry powder below 30°C, protected from light and moisture. Keep out of reach of children. Reconstituted suspension should be stored in a refrigerator (2-8°C) and used within 7-14 days.
Overdose
Oral overdose of colistin sulphate is unlikely to cause systemic toxicity due to poor absorption. However, gastrointestinal symptoms like nausea, vomiting, and diarrhea may be exacerbated. Management is supportive.
Pregnancy & Lactation
Pregnancy Category C. Use only if clearly needed and the potential benefits outweigh the potential risks to the fetus. Colistin is excreted in breast milk; use with caution during lactation or consider an alternative.
