Description
Overview of the medicine
Tanpot 100 mg Tablet contains Tanpotine Hydrochloride, a selective serotonin reuptake inhibitor (SSRI) used for the treatment of major depressive disorder, obsessive-compulsive disorder, panic disorder, and other anxiety disorders. It helps to restore the balance of serotonin, a natural substance in the brain, which improves mood and reduces symptoms of anxiety and panic.
Uses & Indications
Dosage
Adults
Initial dose 50 mg once daily, may be increased to 100 mg after several weeks. Maximum dose is typically 200 mg once daily, depending on response and tolerability.
Elderly
Lower initial doses (e.g., 25 mg) are often recommended, with gradual titration based on response and tolerability due to increased sensitivity and potential for adverse effects.
Renal_impairment
No dosage adjustment is generally required for mild to moderate renal impairment. For severe impairment, caution is advised and a lower starting dose might be considered.
How to Take
Take orally once daily, in the morning or evening, with or without food. Swallow the tablet whole with water; do not crush or chew.
Mechanism of Action
Tanpotine Hydrochloride selectively inhibits the reuptake of serotonin by neurons in the central nervous system, thereby increasing the concentration of serotonin in the synaptic cleft. This potentiation of serotonergic neurotransmission is thought to be responsible for its antidepressant and anxiolytic effects.
Pharmacokinetics
Onset
Therapeutic effects typically begin within 2-4 weeks, though some patients may experience initial improvements earlier.
Excretion
Excreted mainly via urine (approximately 50-60%) and feces (approximately 30-40%) as metabolites.
Half life
Approximately 24-26 hours, allowing for once-daily dosing.
Absorption
Well absorbed orally, with peak plasma concentrations reached in 4-8 hours. Food may slightly delay absorption but does not significantly affect overall bioavailability.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C19 and CYP3A4, into inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Tanpotine Hydrochloride or any component of the tablet
- Concomitant use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of discontinuing an MAOI
- Concomitant use with pimozide
- Uncontrolled narrow-angle glaucoma
Drug Interactions
Pimozide
Increased pimozide levels and cardiotoxicity. Contraindicated.
Warfarin
Increased risk of bleeding due to inhibition of CYP2C19.
Tricyclic Antidepressants (TCAs)
May increase TCA plasma concentrations, requiring dosage adjustment.
Monoamine Oxidase Inhibitors (MAOIs)
Serious, sometimes fatal, reactions including serotonin syndrome. Contraindicated.
Non-steroidal Anti-inflammatory Drugs (NSAIDs) and Aspirin
Increased risk of bleeding, especially gastrointestinal bleeding.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, nausea, vomiting, tachycardia, tremor, agitation, dizziness, and rarely, seizures or serotonin syndrome. Management involves supportive care, maintaining an open airway, monitoring cardiac and vital signs, and symptomatic treatment. Gastric lavage and activated charcoal may be considered soon after ingestion.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Tanpotine is excreted in breast milk; advise caution when administered to a nursing mother. Consult a doctor before use during pregnancy or lactation.
