Description
Overview of the medicine
Tanpot 25 mg Tablet contains Tadalafil, a phosphodiesterase type 5 (PDE5) inhibitor. It is primarily used to treat erectile dysfunction (ED) and symptoms of benign prostatic hyperplasia (BPH). Tadalafil helps to increase blood flow to the penis, enabling an erection when sexually stimulated.
Uses & Indications
Dosage
Adults
For erectile dysfunction (ED): 25 mg as a single dose, taken orally as needed, approximately 30 minutes before sexual activity. Do not take more than once daily. For BPH or ED/BPH: Typically 5 mg once daily. Adjust dosage based on individual response and tolerability, as directed by a physician. (Note: 25 mg is not a standard Tadalafil dose for BPH, consult physician for appropriate regimen).
Elderly
No dosage adjustment required based on age alone. However, increased sensitivity to the drug in some older individuals cannot be ruled out.
Renal_impairment
For patients with severe renal impairment (creatinine clearance <30 mL/min): Maximum dose for on-demand ED is 5 mg, not to exceed once every 72 hours. For daily use, not recommended or requires significant dose reduction as advised by a physician.
How to Take
Take Tanpot 25 mg Tablet orally with or without food. For ED, take it approximately 30 minutes before anticipated sexual activity. For daily use (if prescribed a lower strength), take it at approximately the same time each day.
Mechanism of Action
Tadalafil selectively inhibits cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5), which is responsible for the degradation of cGMP in the corpus cavernosum. By inhibiting PDE5, Tadalafil increases cGMP levels, leading to smooth muscle relaxation and increased blood flow into the penis during sexual stimulation, thereby facilitating an erection. In BPH, it is thought to relax smooth muscle in the bladder and prostate.
Pharmacokinetics
Onset
Within 30-60 minutes.
Excretion
Excreted predominantly as metabolites in the feces (approximately 61% of the dose) and to a lesser extent in the urine (approximately 36% of the dose).
Half life
Approximately 17.5 hours, allowing for efficacy up to 36 hours (often referred to as 'the weekend pill').
Absorption
Rapidly absorbed after oral administration. Mean maximum plasma concentration (Cmax) is achieved within 2 hours.
Metabolism
Primarily metabolized by the cytochrome P450 (CYP) 3A4 isoenzyme to a catechol metabolite, which is then glucuronidated.
Side Effects
Contraindications
- Concomitant use of nitrates (e.g., nitroglycerin, isosorbide dinitrate) due to risk of severe hypotension.
- Hypersensitivity to Tadalafil or any component of the tablet.
- Concomitant use with guanylate cyclase stimulators (e.g., riociguat) due to risk of symptomatic hypotension.
- Patients with severe cardiovascular conditions for whom sexual activity is inadvisable.
Drug Interactions
Nitrates
Contraindicated due to potentiation of hypotensive effects, leading to severe hypotension.
Antihypertensives
May cause additive blood pressure lowering effects.
Alpha-blockers (e.g., doxazosin, tamsulosin)
Potential for additive hypotensive effects, particularly with concomitant use. Initiate with caution.
CYP3A4 inducers (e.g., rifampin, carbamazepine, phenytoin)
Can decrease Tadalafil plasma levels, potentially reducing efficacy.
CYP3A4 inhibitors (e.g., ketoconazole, ritonavir, clarithromycin)
Can increase Tadalafil plasma levels, requiring dose adjustment.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In cases of overdose, standard supportive measures should be adopted as required. Haemodialysis is not expected to accelerate Tadalafil clearance due to its high protein binding and negligible urinary excretion as intact drug. Monitor blood pressure and manage any hypotensive symptoms.
Pregnancy & Lactation
Tanpot (Tadalafil) is not indicated for use in women. There are no adequate and well-controlled studies in pregnant or lactating women. Excretion of Tadalafil into human milk is unknown. Use in women of childbearing potential or breastfeeding is generally not recommended unless specifically indicated for conditions like Pulmonary Arterial Hypertension where benefits outweigh risks.

Clinical Trials
Tadalafil has undergone extensive clinical trials for the treatment of erectile dysfunction, benign prostatic hyperplasia, and pulmonary arterial hypertension, demonstrating its efficacy and safety profile.
Lab Monitoring
- No routine laboratory monitoring is typically required for Tadalafil use. However, patients with underlying cardiovascular disease or renal impairment should be monitored clinically as appropriate.
Doctor Notes
- Counsel patients on the proper administration and potential side effects, emphasizing the importance of not co-administering with nitrates or guanylate cyclase stimulators.
- Assess cardiovascular status before prescribing, as sexual activity itself poses a cardiac risk.
- Advise patients to report priapism, sudden vision changes, or sudden hearing loss immediately.
Patient Guidelines
- Do not take Tanpot 25 mg Tablet if you are taking nitrates for chest pain or heart problems, as it can cause a severe drop in blood pressure.
- Seek immediate medical attention if you experience an erection lasting longer than 4 hours (priapism) or sudden vision or hearing loss.
- Inform your doctor about all your medical conditions and medications, especially if you have heart problems, high or low blood pressure, kidney or liver disease, or a history of priapism.
- Avoid excessive alcohol consumption, as it may increase the risk of dizziness and low blood pressure.
Missed Dose Advice
For on-demand use (ED), a dose is taken only when needed, so there is no missed dose. For daily use (lower strengths, for BPH or daily ED), if you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Tadalafil may cause dizziness or visual disturbances. Patients should be aware of how they react to Tanpot 25 mg Tablet before driving or operating machinery.
Lifestyle Advice
- Maintain a healthy lifestyle, including regular exercise and a balanced diet, to support overall cardiovascular health.
- If experiencing erectile dysfunction, consider reducing stress, improving sleep hygiene, and consulting a healthcare professional for underlying causes.
