Overview of the medicine
Taven-EZ is a fixed-dose combination medication containing Teneligliptin and Ezetimibe. It is used to manage type 2 diabetes mellitus and dyslipidemia, specifically to lower blood glucose and cholesterol levels in patients when diet and exercise alone are not enough.
Uses & Indications
Dosage
Adults
One tablet once daily, preferably at the same time each day.
Elderly
No specific dose adjustment is required based on age alone; monitor renal function.
Renal_impairment
No dose adjustment is necessary for mild to moderate renal impairment. For severe renal impairment, consult a physician.
How to Take
Taven-EZ tablet should be taken orally, once daily, with or without food. It is recommended to take it at the same time each day.
Mechanism of Action
Teneligliptin inhibits Dipeptidyl Peptidase-4 (DPP-4), increasing active incretin hormones (GLP-1 and GIP) to enhance glucose-dependent insulin secretion and suppress glucagon. Ezetimibe inhibits cholesterol absorption in the small intestine, reducing cholesterol delivery to the liver and subsequently lowering LDL-C.

Onset
Teneligliptin: Glucose-lowering effect within hours. Ezetimibe: Lipid-lowering effects observed within 1-2 weeks.
Excretion
Teneligliptin: Primarily renal excretion (60-70%), some fecal. Ezetimibe: Primarily fecal excretion as active glucuronide and unconjugated ezetimibe.
Half life
Teneligliptin: Approximately 24 hours. Ezetimibe: Active glucuronide approximately 22 hours.
Absorption
Teneligliptin is rapidly absorbed (Tmax 1-2 hours). Ezetimibe is rapidly absorbed and extensively metabolized to its active glucuronide (Tmax 4-12 hours for glucuronide).
Metabolism
Teneligliptin: Primarily metabolized by CYP3A4 and FMO3. Ezetimibe: Extensively glucuronidated in the intestine and liver.
Side Effects
Contraindications
- •Hypersensitivity to Teneligliptin, Ezetimibe, or any excipients.
- •Patients with type 1 diabetes mellitus or diabetic ketoacidosis.
- •Patients with severe hepatic impairment (Child-Pugh score >9) for Ezetimibe.
Drug Interactions
Cyclosporine
Coadministration with ezetimibe may increase ezetimibe levels. Monitor cyclosporine concentrations.
Fibrates (e.g., Gemfibrozil)
Co-administration with ezetimibe is not recommended due to potential for increased cholelithiasis.
Bile Acid Sequestrants (e.g., Cholestyramine)
May reduce ezetimibe absorption. Ezetimibe should be taken at least 2 hours before or 4 hours after bile acid sequestrants.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of an overdose, symptomatic and supportive measures should be initiated. There is no specific antidote.
