Description
Overview of the medicine
Taxceph 250 mg Injection contains Ceftriaxone, a third-generation cephalosporin antibiotic. It is used to treat a wide range of bacterial infections including respiratory tract, urinary tract, skin, bone, and joint infections, as well as meningitis and sepsis. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
Typical adult dose for infections is 1-2 g once daily. For uncomplicated gonorrhea, a single 250 mg IM dose is common. Dosage depends on severity and type of infection.
Elderly
No specific dose adjustment is usually required for elderly patients with normal renal and hepatic function.
Renal_impairment
In severe renal impairment (creatinine clearance <10 mL/min), dose should not exceed 2 g once daily. Monitoring of plasma concentrations may be necessary.
How to Take
Taxceph 250 mg injection is administered intravenously (IV) or intramuscularly (IM) after reconstitution with appropriate diluent (e.g., sterile water for injection, lidocaine for IM). IV administration should be given slowly over 30 minutes. IM injections should be given deep into a large muscle mass.
Mechanism of Action
Ceftriaxone is a bactericidal antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to cell lysis and death.
Pharmacokinetics
Onset
Rapid, typically within 1-3 hours.
Excretion
Approximately 50-60% excreted unchanged via urine and 40-50% via bile into feces.
Half life
Approximately 6-9 hours (can be prolonged in neonates and elderly).
Absorption
Rapidly and completely absorbed after intramuscular (IM) administration; 100% bioavailability after intravenous (IV) administration.
Metabolism
Partially metabolized in the gut by intestinal flora; approximately 33-67% eliminated unchanged.
Side Effects
Contraindications
- •Hypersensitivity to ceftriaxone, other cephalosporins, or any component of the formulation.
- •History of severe hypersensitivity reaction to any other type of beta-lactam antibacterial agent (e.g., penicillins, carbapenems).
- •Neonates with hyperbilirubinemia, especially premature neonates.
- •Neonates (≤ 28 days of age) receiving intravenous calcium-containing solutions due to risk of precipitation.
Drug Interactions
Aminoglycosides
Synergistic antibacterial effect; monitor renal function if used concomitantly.
Chloramphenicol
Antagonistic effect observed in vitro; avoid concomitant use.
Calcium-containing solutions
Concomitant use is contraindicated in neonates; in other age groups, administer sequentially with adequate flush.
Loop diuretics (e.g., Furosemide)
May increase the risk of nephrotoxicity with concomitant use.
Oral anticoagulants (e.g., Warfarin)
May increase the risk of bleeding; monitor coagulation parameters.
Storage
Store the powder below 30°C in a dry place, protected from light. Reconstituted solution should be used immediately or stored for a limited time (e.g., 6 hours at room temperature, 24 hours in refrigerator) as per manufacturer guidelines.
Overdose
In case of overdose, symptoms may include nausea, vomiting, diarrhea, and injection site reactions. Treatment is supportive and symptomatic. Ceftriaxone is not significantly removed by hemodialysis or peritoneal dialysis.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Ceftriaxone is excreted in low concentrations in human breast milk; use with caution in nursing mothers.
