Description
Overview of the medicine
Taxceph 500 mg Injection contains Ceftriaxone, a third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
Usual adult dose is 500 mg to 2 g administered once daily, either intravenously (IV) or intramuscularly (IM), depending on the severity of the infection. In some cases, a dose of 2 g twice daily may be used.
Elderly
No specific dose adjustment is required for elderly patients with normal renal and hepatic function. Dosage should be adjusted based on the severity of infection and general condition.
Renal_impairment
No dosage adjustment is generally required for mild to moderate renal impairment. For severe renal impairment or patients undergoing dialysis, careful monitoring and possible dose adjustment may be necessary. Maximum daily dose should not exceed 2g without close monitoring.
How to Take
For intramuscular administration, dissolve 500 mg in 2 ml of lidocaine 1% solution and inject deeply into a large muscle. For intravenous administration, dissolve 500 mg in 5 ml of sterile water for injection and administer slowly over 2-4 minutes. For IV infusion, dilute the reconstituted solution further and infuse over 30 minutes.
Mechanism of Action
Ceftriaxone is a bactericidal antibiotic that inhibits bacterial cell wall synthesis by binding to one or more of the penicillin-binding proteins (PBPs) which are located on the bacterial cell wall. This inhibition leads to the disruption of cell wall integrity and ultimately cell lysis and death of the bacteria.
Pharmacokinetics
Onset
Rapid, typically within minutes of IV administration and within hours of IM administration.
Excretion
Excreted by both renal (kidney) and biliary (liver) pathways.
Half life
Approximately 5.8 to 8.7 hours in healthy adults, allowing for once-daily dosing.
Absorption
Rapidly and completely absorbed after intramuscular (IM) administration, achieving peak plasma concentrations within 2-3 hours. Intravenous (IV) administration results in immediate peak concentrations.
Metabolism
Limited metabolism; approximately 33-67% is excreted unchanged in the urine, and the remainder is excreted via bile.
Side Effects
Contraindications
- •Known hypersensitivity to ceftriaxone, any other cephalosporin, or to any of the excipients.
- •Patients with previous immediate and severe hypersensitivity to penicillin or any other beta-lactam antibiotics.
- •Neonates (≤ 28 days) with hyperbilirubinemia, particularly premature neonates, due to the risk of bilirubin encephalopathy.
- •Concomitant use with calcium-containing IV solutions in neonates (≤ 28 days).
Drug Interactions
Aminoglycosides
May increase the risk of nephrotoxicity.
Calcium-containing solutions
Concomitant use is contraindicated in neonates due to risk of precipitation. In other age groups, if calcium-containing solutions are necessary, administer ceftriaxone and calcium solutions sequentially with thorough flushing of the IV line.
Loop diuretics (e.g., Furosemide)
High doses may increase the risk of nephrotoxicity.
Oral Anticoagulants (e.g., Warfarin)
May increase the anticoagulant effect, requiring close monitoring of INR and dose adjustment.
Storage
Store below 30°C in a dry place, away from light. Do not freeze. Reconstituted solution should be used immediately or within 24 hours if refrigerated.
Overdose
In case of overdose, symptoms may include nausea, vomiting, and diarrhea. Management is primarily symptomatic and supportive. Hemodialysis or peritoneal dialysis will not aid in the elimination of ceftriaxone.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. It should be used during pregnancy only if clearly needed. Ceftriaxone is excreted in low concentrations into breast milk; caution should be exercised when administered to a nursing woman.
