Overview of the medicine
Taxetil DS contains Cefixime, a third-generation cephalosporin antibiotic used to treat various bacterial infections. Its 'DS' (Double Strength) formulation provides 400 mg of Cefixime per tablet, offering convenient once-daily dosing for many indications.
Uses & Indications
Dosage
Adults
The recommended adult dose is 400 mg orally once daily or 200 mg orally every 12 hours.
Elderly
No dosage adjustment is generally required in elderly patients, unless renal function is significantly impaired.
Renal_impairment
Dosage adjustment is necessary in patients with creatinine clearance less than 60 mL/min. For CrCl 21-59 mL/min, 300 mg daily; for CrCl < 20 mL/min, 200 mg daily.
How to Take
Taxetil DS tablets should be swallowed whole with water. It can be taken with or without food. For optimal absorption and to minimize gastrointestinal discomfort, it is sometimes advised to take it with food.
Mechanism of Action
Cefixime is a bactericidal antibiotic that acts by inhibiting bacterial cell wall synthesis. It selectively binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the cross-linking of peptidoglycan chains essential for cell wall strength and integrity, leading to cell lysis and bacterial death.
Pharmacokinetics
Onset
Bactericidal effects typically begin within a few hours of the first dose.
Excretion
Primarily excreted unchanged in the urine (about 50-60% of the absorbed dose) and also via biliary excretion. Renal impairment significantly prolongs its elimination.
Half life
The serum half-life ranges from 3 to 4 hours, which allows for once- or twice-daily dosing.
Absorption
Cefixime is approximately 40-50% absorbed orally, with or without food. Peak serum concentrations are achieved between 2-6 hours after administration.
Metabolism
Cefixime is not extensively metabolized in the body. Only a small fraction appears to undergo hepatic metabolism.
Side Effects
Contraindications
- Hypersensitivity to Cefixime or any other cephalosporin antibiotics.
- History of serious hypersensitivity reactions to penicillin (due to potential cross-reactivity).
Drug Interactions
Warfarin
May increase prothrombin time (PT) and INR, increasing risk of bleeding. Close monitoring is advised.
Nifedipine
May increase cefixime peak plasma levels and AUC.
Carbamazepine
Increased carbamazepine levels have been reported when co-administered with cefixime.
Oral Contraceptives
Reduced efficacy of oral contraceptives has been reported with some antibiotics, including cephalosporins, though evidence is limited for cefixime.

Store below 30°C, in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities from the circulation by hemodialysis or peritoneal dialysis. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Cefixime is excreted in small amounts in breast milk; caution should be exercised when administered to a nursing mother.
