Description
Overview of the medicine
Telide 200 mg/5 ml Suspension contains Azithromycin, a macrolide antibiotic used to treat various bacterial infections, including respiratory tract infections, skin and soft tissue infections, and certain sexually transmitted infections. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
For most infections: 500 mg (12.5 ml of 200 mg/5 ml suspension) as a single dose on day 1, then 250 mg (6.25 ml) once daily on days 2-5, or 500 mg once daily for 3 days. For Chlamydia: 1 g (25 ml) as a single oral dose.
Elderly
No specific dosage adjustment is required for elderly patients.
Renal_impairment
No dosage adjustment is needed for mild to moderate renal impairment (creatinine clearance > 10 ml/min). Use with caution in severe renal impairment.
How to Take
Shake the bottle well before each use. Administer orally using the provided measuring spoon or cup. Can be taken with or without food. Complete the full prescribed course of treatment.
Mechanism of Action
Azithromycin inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit of susceptible microorganisms, thereby preventing translocation of peptide chains.
Pharmacokinetics
Onset
Clinical effect typically observed within 2-3 days of initiation, though plasma peak concentration occurs within 2-3 hours.
Excretion
Mainly excreted unchanged in bile; a small percentage is excreted in urine.
Half life
Terminal elimination half-life is 2-4 days due to extensive tissue uptake and slow release.
Absorption
Rapidly absorbed from the gastrointestinal tract; bioavailability is approximately 37%. Peak plasma concentrations are reached within 2-3 hours.
Metabolism
Primarily metabolized in the liver, largely to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to azithromycin, erythromycin, any macrolide or ketolide antibiotic, or to any component of the formulation.
- History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use.
Drug Interactions
Digoxin
Increased serum digoxin levels. Monitor digoxin levels.
Warfarin
Increased anticoagulant effect. Monitor INR closely.
Nelfinavir
Increased azithromycin serum concentrations. No dose adjustment needed, but monitor for side effects.
Cyclosporine
Increased cyclosporine levels. Monitor cyclosporine levels and adjust dose as necessary.
Antacids containing aluminum or magnesium
May decrease peak serum levels of azithromycin. Administer azithromycin at least 1-2 hours before or after antacids.
Storage
Store unreconstituted powder below 30°C in a dry place, away from light and moisture. Keep out of reach of children. Store reconstituted suspension in a refrigerator (2-8°C) and discard any unused portion after 5-10 days.
Overdose
Symptoms of overdose typically include severe nausea, vomiting, diarrhea, and reversible hearing loss. Management involves gastric lavage and general symptomatic and supportive measures as clinically indicated.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. However, use in pregnancy only if clearly needed. Azithromycin is excreted in human milk; use with caution in nursing mothers.
