Description
Overview of the medicine
Telide 500 mg Tablet contains Azithromycin, a macrolide antibiotic used to treat various bacterial infections, including respiratory tract infections, skin and soft tissue infections, and sexually transmitted diseases. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
Typical dose for most infections is 500 mg once daily for 3 days, or 500 mg once on day 1 followed by 250 mg once daily for 4 days. For STIs, a single 1 g dose may be used.
Elderly
No dosage adjustment is generally required for elderly patients with normal renal and hepatic function.
Renal_impairment
No dosage adjustment is necessary for mild to moderate renal impairment. Caution is advised for severe renal impairment (CrCl <10 mL/min) due to lack of data.
How to Take
Telide 500 mg Tablet can be taken with or without food. To minimize gastrointestinal upset, it may be taken with food. Swallow the tablet whole with a glass of water. Do not crush or chew.
Mechanism of Action
Azithromycin acts by binding to the 50S ribosomal subunit of susceptible microorganisms and, thus, interfering with microbial protein synthesis. It is generally considered bacteriostatic but can be bactericidal at high concentrations or against highly susceptible organisms.

Onset
Peak plasma concentrations are reached within 2-3 hours.
Excretion
Mainly excreted unchanged in the bile, with a small amount excreted in urine (approx. 6%).
Half life
Terminal elimination half-life of 2-4 days (40-68 hours).
Absorption
Well absorbed from the gastrointestinal tract; bioavailability is approximately 37% and is slightly reduced by food.
Metabolism
Primarily metabolized in the liver by N-demethylation.

Contraindications
- Hypersensitivity to Azithromycin, Erythromycin, or any macrolide or ketolide antibiotic
- History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use
Drug Interactions
Warfarin
Increased anticoagulant effect, monitor INR.
Nelfinavir
Increased azithromycin serum concentrations; monitor for side effects.
Antacids containing aluminum or magnesium
Decreased peak plasma levels of azithromycin; take azithromycin at least 1 hour before or 2 hours after antacids.
QT-prolonging drugs (e.g., antiarrhythmics, antipsychotics)
Increased risk of QT prolongation and cardiac arrhythmias.
