Description
Overview of the medicine
Telmicard 40 mg Tablet contains Telmisartan, an angiotensin II receptor blocker (ARB) used to treat high blood pressure (hypertension) and to reduce the risk of cardiovascular events like heart attack or stroke in patients at high risk.
Uses & Indications
Dosage
Adults
The usual recommended dose is 40 mg once daily. If blood pressure is not controlled, the dose may be increased to a maximum of 80 mg once daily.
Elderly
No dosage adjustment is generally needed for elderly patients.
Renal_impairment
No dosage adjustment is required for patients with mild to moderate renal impairment. Caution is advised in severe renal impairment.
How to Take
Telmicard tablets should be taken orally once daily with or without food. It should be swallowed with water.
Mechanism of Action
Telmisartan selectively blocks the binding of angiotensin II to the AT1 receptor in various tissues, including vascular smooth muscle and the adrenal gland. This blockade inhibits the vasoconstrictor and aldosterone-secreting effects of angiotensin II, leading to vasodilation and reduction in blood pressure.
Pharmacokinetics
Onset
Antihypertensive effect starts within 3 hours. Maximum blood pressure reduction is generally attained after 4 to 8 weeks of therapy.
Excretion
Almost exclusively eliminated in feces (approximately 97%) via biliary excretion. Less than 1% is excreted in urine.
Half life
Approximately 24 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract. Absolute bioavailability is dose-dependent, approximately 42% for a 40 mg dose.
Metabolism
Primarily metabolized by conjugation to form an acylglucuronide, which is pharmacologically inactive. Minimal metabolism by cytochrome P450 system.
Side Effects
Contraindications
- Hypersensitivity to the active substance or to any of the excipients.
- Second and third trimesters of pregnancy.
- Biliary obstructive disorders.
- Severe hepatic impairment.
- Concomitant use of aliskiren in patients with diabetes mellitus or renal impairment (GFR < 60 mL/min/1.73 m²).
Drug Interactions
Digoxin
Increased plasma concentrations of digoxin may occur; monitor digoxin levels.
Lithium
Increased serum lithium concentrations and toxicity have been reported. Monitor lithium levels.
Aliskiren
Concomitant use is contraindicated in diabetic or renally impaired patients due to increased risk of hyperkalemia, hypotension, and renal impairment.
NSAIDs (Non-Steroidal Anti-Inflammatory Drugs)
May reduce the antihypertensive effect of Telmisartan and increase the risk of renal impairment.
Potassium-sparing diuretics, potassium supplements, or other agents that increase potassium levels
May lead to hyperkalemia. Monitor serum potassium levels.
Storage
Store in a dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
The most likely manifestations of overdose would be hypotension, dizziness, and tachycardia; bradycardia could occur from parasympathetic stimulation. Treatment is symptomatic and supportive. Telmisartan is not removed by hemodialysis.
Pregnancy & Lactation
Telmicard is contraindicated during the second and third trimesters of pregnancy and should not be used in the first trimester. It is not recommended during breastfeeding.
