Overview of the medicine
Telmicard 80 mg Tablet contains Telmisartan, an angiotensin II receptor blocker (ARB), used to treat high blood pressure (hypertension) and to reduce the risk of cardiovascular events in patients aged 55 years or older at high risk.
Uses & Indications
Dosage
Adults
The usual recommended dose is 40 mg once daily. For Telmicard 80 mg, it can be used when 40 mg is insufficient, or for cardiovascular risk reduction. Max dose is 80 mg once daily.
Elderly
No dosage adjustment is generally necessary for elderly patients.
Renal_impairment
No dosage adjustment is required for patients with mild to moderate renal impairment. For severe impairment, monitoring is advised. Not recommended for patients on dialysis.
How to Take
Telmicard 80 mg tablet can be taken with or without food, preferably at the same time each day.
Mechanism of Action
Telmisartan selectively blocks the binding of angiotensin II to the AT1 receptor in various tissues (e.g., vascular smooth muscle, adrenal gland). This blockade inhibits the vasoconstrictive and aldosterone-secreting effects of angiotensin II, leading to vasodilation and reduced blood pressure.
Pharmacokinetics
Onset
Antihypertensive effect begins within 3 hours; full effect usually achieved within 4-8 weeks.
Excretion
Primarily excreted unchanged in feces (greater than 97%), with less than 1% excreted in urine.
Half life
Approximately 20-24 hours, allowing for once-daily dosing.
Absorption
Rapidly absorbed from the gastrointestinal tract, though the extent of absorption is variable (approximately 42-58%). Peak plasma concentrations are reached within 0.5-1 hour.
Metabolism
Metabolized by conjugation to form a pharmacologically inactive acylglucuronide; not extensively metabolized by cytochrome P450 isoenzymes.
Side Effects
Contraindications
- Hypersensitivity to telmisartan or any component of the formulation.
- Second and third trimesters of pregnancy.
- Biliary obstructive disorders.
- Severe hepatic impairment.
- Concomitant use with aliskiren in patients with diabetes mellitus or renal impairment (GFR < 60 mL/min).
Drug Interactions
Digoxin
Increased plasma digoxin concentrations.
Lithium
Increased serum lithium concentrations and toxicity.
Aliskiren
Contraindicated in diabetic or renally impaired patients due to increased risk of hyperkalemia, hypotension, and renal impairment.
Diuretics
Increased risk of hypotension and renal impairment.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce the antihypertensive effect of telmisartan and increase the risk of renal impairment.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
The most likely manifestation of overdose would be hypotension, dizziness, and tachycardia; bradycardia could occur from parasympathetic stimulation. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category D (2nd and 3rd trimesters) due to fetal toxicity; contraindicated. Not recommended during lactation as it is unknown if telmisartan is excreted in human milk.
