Overview of the medicine
Telnor 40 mg Tablet contains Telmisartan, an angiotensin II receptor blocker (ARB) used to treat high blood pressure (hypertension) and to reduce the risk of cardiovascular events in certain high-risk patients.
Uses & Indications
Dosage
Adults
The usual recommended dose is 40 mg once daily. The dose may be increased to a maximum of 80 mg once daily if needed.
Elderly
No dosage adjustment is generally needed for elderly patients.
Renal_impairment
No dosage adjustment is required for patients with mild to moderate renal impairment. For severe renal impairment or hemodialysis, caution is advised and lower starting doses may be considered.
How to Take
Telnor tablet should be swallowed whole with some water, with or without food. It should be taken at approximately the same time each day.
Mechanism of Action
Telmisartan selectively blocks the binding of angiotensin II to the AT1 receptor, which is found in many tissues, including vascular smooth muscle and the adrenal gland. This blockade inhibits the vasoconstrictive and aldosterone-secreting effects of angiotensin II, leading to vasodilation, decreased blood pressure, and reduced sodium and water retention.

Onset
Within 3 hours, full effect achieved within 4-8 weeks.
Excretion
Mainly excreted unchanged in feces via biliary excretion (approx 98%).
Half life
Approximately 24 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract; absolute bioavailability is dose-dependent, approx 42-58%.
Metabolism
Minimally metabolized; primarily conjugated to its acylglucuronide.
Side Effects
Contraindications
- •Hypersensitivity to the active substance or to any of the excipients.
- •Second and third trimesters of pregnancy.
- •Biliary obstructive disorders.
- •Severe hepatic impairment.
- •Concomitant use with aliskiren in patients with diabetes mellitus or renal impairment (GFR < 60 mL/min/1.73 m²).
Drug Interactions
Digoxin
Increased plasma levels of digoxin.
Lithium
Increased serum lithium concentrations and toxicity.
Diuretics (e.g., furosemide)
Increased risk of hypotension and renal impairment.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
Reduced antihypertensive effect and increased risk of renal impairment, including acute renal failure.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
The most likely manifestation of overdose is hypotension, dizziness, and tachycardia; bradycardia could also occur. Treatment is symptomatic and supportive. Telmisartan is not removed by hemodialysis.
Pregnancy & Lactation
Contraindicated during the second and third trimesters of pregnancy due to risk of fetal injury. Not recommended during breastfeeding as it is unknown if Telmisartan is excreted in human milk.
