Overview of the medicine
Telmisartan is an angiotensin II receptor blocker used to treat high blood pressure and reduce the risk of cardiovascular events such as heart attack and stroke.
Uses & Indications
Dosage
Adults
For hypertension, usual starting dose is 40 mg once daily. Max dose 80 mg once daily. For cardiovascular risk reduction, 80 mg once daily.
Elderly
No dosage adjustment needed based on age. Consider lower starting dose (20 mg) if clinically indicated.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment. For severe renal impairment or dialysis, caution is advised.
How to Take
Taken orally, with or without food, once daily.
Mechanism of Action
Telmisartan selectively blocks the binding of angiotensin II to the AT1 receptor, primarily located in vascular smooth muscle and the adrenal gland. This inhibition prevents the vasoconstrictor and aldosterone-secreting effects of angiotensin II, leading to vasodilation and reduced blood pressure.

Onset
Antihypertensive effect within 3 hours, full effect in 4-8 weeks.
Excretion
Primarily excreted unchanged in feces via biliary excretion (over 97%).
Half life
Approximately 24 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract. Absolute bioavailability is dose-dependent (approx. 42-58%). Food reduces bioavailability slightly but is not clinically significant.
Metabolism
Minimally metabolized by conjugation to glucuronide; inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to telmisartan or any component of the formulation
- •Pregnancy (especially second and third trimesters)
- •Lactation
- •Biliary obstructive disorders
- •Severe hepatic impairment
- •Concomitant use with aliskiren in patients with diabetes mellitus or renal impairment (GFR < 60 mL/min/1.73 m²)
Drug Interactions
NSAIDs
May reduce the antihypertensive effect and increase the risk of renal impairment.
Digoxin
Increased plasma digoxin concentrations.
Lithium
Increased serum lithium concentrations and toxicity.
Aliskiren
Increased risk of hyperkalemia, hypotension, and renal impairment in patients with diabetes or renal impairment.
Diuretics
Increased hypotensive effect.
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children.
Overdose
Symptoms may include significant hypotension, dizziness, and tachycardia or bradycardia. Treatment is supportive and symptomatic; hemodialysis is not effective.
Pregnancy & Lactation
Contraindicated during pregnancy (especially second and third trimesters) due to risk of fetal injury or death. Not recommended during lactation as telmisartan is excreted in breast milk.
