Description
Overview of the medicine
Telsan (Telmisartan) is an angiotensin II receptor blocker (ARB) used to treat high blood pressure (hypertension) and to reduce the risk of cardiovascular events in certain high-risk patients.
Uses & Indications
Dosage
Adults
Usually 40 mg once daily. May be increased to a maximum of 80 mg once daily if needed.
Elderly
No dosage adjustment is generally required for elderly patients.
Renal_impairment
No dosage adjustment required for mild to moderate renal impairment. For severe impairment, caution is advised.
How to Take
Oral administration, once daily, with or without food.
Mechanism of Action
Telmisartan selectively blocks the binding of angiotensin II to the AT1 receptor, which prevents vasoconstriction and aldosterone secretion, leading to a reduction in blood pressure.
Pharmacokinetics
Onset
Peak plasma concentrations are reached in 0.5-1 hour.
Excretion
Primarily excreted unchanged in feces (over 97%), with minimal renal excretion.
Half life
Approximately 24 hours.
Absorption
Rapidly absorbed, with absolute bioavailability ranging from 42% (40 mg) to 58% (160 mg).
Metabolism
Undergoes conjugation to form a pharmacologically inactive acylglucuronide.

Contraindications
- Hypersensitivity to telmisartan or any component of the product
- Pregnancy (second and third trimesters)
- Lactation
- Biliary obstructive disorders
- Severe hepatic impairment
- Concomitant use with aliskiren in patients with diabetes mellitus or renal impairment (GFR < 60 mL/min/1.73 m²)
Drug Interactions
NSAIDs
Reduced antihypertensive effect, potential for increased renal impairment.
Digoxin
Increased plasma concentrations of digoxin.
Lithium
Increased serum lithium concentrations and toxicity.
Diuretics
Enhanced hypotensive effect.

Symptoms of overdose include hypotension, dizziness, and tachycardia. Treatment is symptomatic and supportive. Telmisartan is not removed by hemodialysis.
