Description
Overview of the medicine
Telsar 40 mg Tablet contains Telmisartan, an angiotensin II receptor blocker (ARB) used to treat high blood pressure (hypertension) and to reduce the risk of cardiovascular events like heart attack and stroke in patients at high risk.
Uses & Indications
Dosage
Adults
The usual recommended dose is 40 mg once daily. The maximum dose is 80 mg once daily.
Elderly
No dosage adjustment is generally required for elderly patients.
Renal_impairment
No dosage adjustment is required for patients with mild to moderate renal impairment. For severe renal impairment or hemodialysis patients, careful monitoring is advised.
How to Take
Take Telsar 40 mg Tablet orally once daily with or without food. Swallow the tablet whole with water. Do not chew or crush it. It is recommended to take the tablet at approximately the same time each day.
Mechanism of Action
Telmisartan selectively blocks the binding of angiotensin II to the AT1 receptor in various tissues (e.g., vascular smooth muscle, adrenal gland). This blockade inhibits the vasoconstrictor and aldosterone-secreting effects of angiotensin II, leading to vasodilation, decreased blood pressure, and reduced sodium and water retention.
Pharmacokinetics
Onset
Onset of antihypertensive effect within 3 hours; maximal reduction in blood pressure generally achieved after 4-8 weeks of therapy.
Excretion
Almost exclusively excreted in the feces, mainly as unchanged drug (>97%) and a small amount as glucuronide conjugate. Less than 1% is excreted in the urine.
Half life
Approximately 24 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract; absolute bioavailability is dose-dependent (~42% for 40 mg, ~58% for 80 mg). Peak plasma concentrations occur 0.5-1 hour after oral administration.
Metabolism
Primarily metabolized by conjugation to form an acylglucuronide, which is pharmacologically inactive. Not metabolized by cytochrome P450 system.
Side Effects
Contraindications
- Hypersensitivity to Telmisartan or any component of the tablet.
- Pregnancy (especially during the second and third trimesters).
- Lactation (breastfeeding).
- Biliary obstructive disorders.
- Severe hepatic impairment.
- Concomitant use with aliskiren in patients with diabetes mellitus or renal impairment (GFR <60 ml/min/1.73 m²).
Drug Interactions
Digoxin
Increased plasma digoxin concentration has been reported with concomitant use of Telmisartan.
Lithium
Increased serum lithium concentrations and toxicity have been reported with concomitant use of ARBs.
Ramipril or Aliskiren
Concomitant use is associated with an increased risk of hypotension, hyperkalemia, and decreased renal function, especially in diabetics or those with renal impairment.
Diuretics and other Antihypertensives
May enhance the hypotensive effect of Telmisartan.
Non-Steroidal Anti-Inflammatory Drugs (NSAIDs)
May reduce the antihypertensive effect of Telmisartan and increase the risk of renal impairment.

Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Limited data are available regarding overdose in humans. The most likely manifestations would be hypotension (low blood pressure) and tachycardia; bradycardia could also occur. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Telsar is contraindicated during pregnancy, especially in the second and third trimesters, due to the risk of fetal injury or death. It is also not recommended during lactation, as it is unknown whether Telmisartan is excreted in human milk.
