Overview of the medicine
Temozar (Temozolomide) is an oral alkylating agent used for certain brain cancers, specifically newly diagnosed glioblastoma multiforme and refractory anaplastic astrocytoma.
Uses & Indications
Dosage
Adults
Highly individualized based on Body Surface Area (BSA) and treatment regimen. E.g., 75 mg/m² daily for 42 days with radiotherapy, then 150-200 mg/m² for 5 days every 28 days.
Elderly
Similar to adults, but close monitoring for myelosuppression is essential.
Renal_impairment
No specific dosage adjustment recommended for mild-moderate renal impairment, but caution is advised.
How to Take
Take orally on an empty stomach (at least 1 hour before or 2 hours after food) to reduce nausea/vomiting. Swallow capsules whole with water; do not open, chew, or crush.
Mechanism of Action
It is an imidazotetrazine derivative, a prodrug that is rapidly converted to its active metabolite, MTIC (5-(3-methyltriazen-1-yl)imidazole-4-carboxamide), which is a cytotoxic alkylating agent. MTIC methylates DNA, primarily at O6 of guanine, leading to DNA damage and apoptosis.

Excretion
Primarily renal (approximately 38% of dose excreted in urine over 7 days, with 5% as unchanged drug).

Absorption

Spontaneous chemical conversion to MTIC at physiological pH, then further metabolized. Minimal hepatic metabolism.
