Tempanil (Paracetamol) is a commonly used medication for relieving mild to moderate pain and reducing fever. It works by blocking the production of certain natural substances in the body that cause pain and fever.
Uses & Indications
Dosage
Adults
Oral: 500 mg to 1000 mg every 4-6 hours as needed. Maximum 4000 mg in 24 hours.
Elderly
Similar to adult dosage, but caution should be exercised in those with impaired renal or hepatic function. Dosing interval may need to be extended.
Renal_impairment
If creatinine clearance is 10-50 mL/min, administer every 6 hours. If less than 10 mL/min, administer every 8 hours.
How to Take
Tempanil tablets can be taken orally with or without food. It is recommended to take them with a glass of water.
Mechanism of Action
Paracetamol primarily acts by inhibiting prostaglandin synthesis in the central nervous system (CNS), leading to analgesic and antipyretic effects. It has minimal anti-inflammatory activity.

Onset
30-60 minutes for analgesic/antipyretic effects.
Excretion
Mainly excreted in urine as glucuronide and sulfate conjugates. Less than 5% is excreted unchanged.
Half life
1-4 hours in adults; slightly longer in neonates and in patients with hepatic or renal impairment.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract; peak plasma concentrations occur within 30-60 minutes.
Metabolism
Primarily metabolized in the liver via glucuronidation and sulfation pathways. A small amount is metabolized by cytochrome P450 to a highly reactive intermediate (NAPQI), which is normally detoxified by conjugation with glutathione.
