Description
Overview of the medicine
Tenaxit (Levofloxacin) is a synthetic broad-spectrum antibacterial agent of the fluoroquinolone class, active against a wide range of gram-positive and gram-negative bacteria.
Uses & Indications
Dosage
Adults
Typically 250 mg to 750 mg once daily, depending on infection type and severity, for 7-14 days.
Elderly
No dosage adjustment needed unless renal function is impaired. Monitor renal function.
Renal_impairment
Dosage adjustment required based on creatinine clearance. For CrCl 20-49 mL/min: 250 mg every 48 hours; for CrCl <20 mL/min: 250 mg initial dose, then 125 mg every 48 hours.
How to Take
Can be taken with or without food. Swallow tablets whole with water. Do not crush or chew.
Mechanism of Action
Inhibits bacterial DNA gyrase and topoisomerase IV, essential enzymes involved in bacterial DNA replication, transcription, repair, and recombination, leading to bacterial cell death.

Onset
Peak plasma concentrations usually attained within 1-2 hours.
Excretion
Mainly excreted in urine as unchanged drug.
Half life
Approximately 6-8 hours.
Absorption
Rapid and complete absorption after oral administration; bioavailability ~99%.
Metabolism
Minimally metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- •Hypersensitivity to levofloxacin or other quinolones
- •History of tendinitis or tendon rupture associated with fluoroquinolone use
- •Epilepsy
- •Children and adolescents (under 18 years)
Drug Interactions
NSAIDs
Increased risk of CNS stimulation and convulsive seizures.
Warfarin
Increased anticoagulant effect; monitor INR.
Antacids (magnesium/aluminum), sucralfate, iron supplements, multivitamins (zinc)
May reduce absorption; administer Tenaxit 2 hours before or 2 hours after these agents.
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children.
Overdose
Symptoms may include CNS effects (confusion, dizziness, convulsions), gastrointestinal reactions (nausea, vomiting), and tendon disorders. Management: symptomatic and supportive measures. Gastric lavage and activated charcoal may be considered. Hemodialysis is not effective.
Pregnancy & Lactation
Category C in pregnancy. Not recommended during pregnancy and lactation unless clearly necessary, due to potential risks to the fetus/infant. Consult a doctor.
