Description
Overview of the medicine
Terbinafine is an oral antifungal medication used to treat fungal infections of the skin, hair, and nails, especially those caused by dermatophytes.
Uses & Indications
Dosage
Adults
250 mg once daily. Duration of treatment varies depending on the site and severity of infection (e.g., Tinea pedis 2-6 weeks, Tinea corporis/cruris 2-4 weeks, Onychomycosis 6-12 weeks).
Elderly
No evidence to suggest a different dosage is needed in elderly patients with normal renal/hepatic function. Caution should be exercised if pre-existing hepatic or renal impairment is present.
Renal_impairment
In patients with creatinine clearance < 50 mL/min or plasma creatinine > 300 µmol/L, the dose should be reduced by 50%.
How to Take
Terbiderm 250 mg tablet should be taken orally with or without food. It is recommended to take the medication at the same time each day.
Mechanism of Action
Terbinafine inhibits squalene epoxidase, an enzyme in the fungal ergosterol biosynthesis pathway. This leads to an accumulation of squalene within the fungal cell and a deficiency of ergosterol, both of which are toxic to the fungal cell and lead to cell death.
Pharmacokinetics
Onset
Accumulates in skin, hair, and nails. Clinical effects onychomycosis take several weeks to months due to the growth rate of nails.
Excretion
Approximately 70% of the dose is excreted in the urine as metabolites. Renal clearance is reduced in patients with renal impairment.
Half life
Terminal half-life is approximately 200-400 hours, reflecting slow elimination from skin and adipose tissue. Initial elimination half-life is 17 hours.
Absorption
Well absorbed from the gastrointestinal tract (>70%). Food may slightly increase bioavailability.
Metabolism
Extensively metabolized in the liver by at least seven CYP isoenzymes, primarily CYP2C9, CYP1A2, CYP3A4, CYP2C8, and CYP2D6. Metabolites are not active.

Contraindications
- •Hypersensitivity to terbinafine or any of its excipients
- •Patients with chronic or active liver disease
- •Severe renal impairment (creatinine clearance < 50 mL/min)
Drug Interactions
Warfarin
Altered prothrombin time/INR observed in patients co-administered terbinafine and warfarin.
Cimetidine
Increases plasma concentration of terbinafine by inhibiting its metabolism.
Rifampicin
Decreases plasma concentration of terbinafine by inducing its metabolism.
Tricyclic antidepressants (TCAs), Selective serotonin reuptake inhibitors (SSRIs), Beta-blockers
Terbinafine inhibits CYP2D6, potentially increasing plasma levels of drugs primarily metabolized by this enzyme.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include headache, nausea, epigastric pain, and dizziness. In case of overdose, gastric lavage and symptomatic supportive treatment are recommended.
Pregnancy & Lactation
Terbinafine is not recommended during pregnancy or lactation unless the potential benefits outweigh the potential risks to the fetus or infant. It is excreted into breast milk.
