Terbitac 250 mg Tablet is an antifungal medication containing Terbinafine. It is used to treat various fungal infections of the skin, hair, and nails. It works by preventing the growth of fungi.
Uses & Indications
Dosage
Adults
Onychomycosis: 250 mg once daily for 6-12 weeks. Tinea pedis/cruris/corporis: 250 mg once daily for 2-4 weeks (may extend to 6 weeks for plantar type tinea pedis).
Elderly
No specific dosage adjustment is generally required, but caution is advised in elderly patients with pre-existing hepatic or renal impairment.
Renal_impairment
In patients with creatinine clearance <50 mL/min or serum creatinine >300 micromol/L, consider a 50% reduction in dose (e.g., 125 mg once daily).
Hepatic_impairment
Terbinafine is contraindicated in patients with chronic or active liver disease. Use with caution at a reduced dose (e.g., 125 mg once daily) in mild to moderate hepatic impairment, with careful monitoring of LFTs.
How to Take
Terbitac 250 mg tablets should be taken orally once daily, with or without food. It is recommended to take the tablet at approximately the same time each day.
Mechanism of Action
Terbinafine is an allylamine antifungal that selectively inhibits fungal squalene epoxidase. This leads to a deficiency of ergosterol (an essential component of the fungal cell membrane) and an accumulation of squalene, resulting in fungal cell death.

Onset
Clinical improvement observed within weeks, full nail clearance may take several months.
Excretion
Mainly excreted in urine (approximately 70-80%) as metabolites, with a small portion excreted in feces.
Half life
Approximately 200-400 hours (terminal half-life) or 20-30 hours (effective half-life).
Absorption
Well absorbed from the GI tract (>70%) but undergoes significant first-pass metabolism, resulting in approximately 40% systemic bioavailability.
Metabolism
Extensively metabolized in the liver by at least 7 CYP isoenzymes, primarily CYP2C9, CYP1A2, CYP3A4, CYP2C8, and CYP2D6.

Contraindications

Drug Interactions
Caffeine
Terbinafine decreases the clearance of intravenously administered caffeine by 19%.
Warfarin
Terbinafine may increase or decrease the anticoagulant effect of warfarin. INR monitoring is recommended.
Cimetidine
Increases plasma concentration of terbinafine by inhibiting CYP2D6 metabolism.
Rifampicin
Decreases plasma concentration of terbinafine by inducing CYP enzymes.
Oral Contraceptives
Irregular menstruation and breakthrough bleeding have been reported, though a causal relationship is not established.
Tricyclic Antidepressants (e.g., Desipramine)
Terbinafine inhibits CYP2D6, potentially increasing plasma levels of co-administered drugs metabolized by this enzyme.

Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include headache, nausea, epigastric pain, and dizziness. In case of overdose, symptomatic and supportive treatment should be initiated. Activated charcoal may be administered to reduce absorption.
Pregnancy & Lactation
Pregnancy Category B. Terbinafine should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in breast milk; therefore, mothers should not breastfeed while taking Terbinafine.
